非综合性唇腭裂危险因素的条件logistic回归分析

摘　　要：目的探索非综合征性唇腭裂患儿孕母早期接触的环境危险因素对唇腭裂发生的影响。方法采用1∶1配对病例对照研究,对母亲孕早期接触的环境危险因素做条件logistic回归。采用EpiData 3.0建立数据库,应用SPSS 19.0软件统计、分析数据。结果共收集252份问卷,单因素logistic回归分析得到22个因素有统计学意义。多因素条件logistic回归分析得到7个因素有统计学意义:母亲孕龄(P=0.04,OR=0.92),文化程度(P<0.01,OR=11.86),孕早期叶酸补充史(P=0.01,OR=0.21),孕期钙补充史(P<0.01,OR=0.15),吸烟及被动吸烟史(P<0.01,OR=7.62),居住房屋新装修(半年之内)史(P=0.01,OR=5.41)。结论为预防非综合性唇腭裂孕妇尽量避免吸二手烟,孕前3个月定时定量补充叶酸、钙等,避免孕早期精神压力过大。Objective The aim of this study was to explore the relationship between the incidence of nonsyndromic cleft lip and palate(NSCLP)and environmental risk factors to which women might be exposed in early pregnancy.Methods A 1:1 matched case-control study method was used.The relationships between NSCLP and various environmental risk factors wereanalyzed by conditional logistic regression.EpiData 3.0 was used to build the database,and SPSS 13.0 statistics was used to analyze the data.Results A total of 22 environmental factors was found by conditional single-factor logistic regression analysis.And 7 environmental factors were found by conditional multi-factor logistic regression analysis,including mother gestational age(P=0.04,OR=0.92),education(P<0.01,OR=11.86),the history of early pregnancy folic acid(P=0.01,OR=0.21),the history of calcium supplement during pregnancy(P<0.01,OR=0.15),history of smoking and passive smoking(P<0.01,OR=7.62),and new residential house decoration(within half a year)(P=0.01,OR=5.41).Conclusion Exposure to tobacco smoke,including second-hand smoke,low living standards,low folic acid and calcium levels,and familial and other stress in the first trimester of pregnancy were all found to be significant determinants in the occurrence of NSCLP.
