摘 要:目的探讨心房颤动(简称房颤)患者左房传导速度随左房双极电位振幅(电压)的变化趋势。方法40例患者行房颤导管消融手术,术中进行左房高密度激动及电压标测。根据左房不同结构、电压,将左房划分为六个结构区域及四个电压区域。分别计算上述各区域的传导速度,并计算不同患者各电压区域的平均传导速度及对应的电压。结果11例患者不满足计算传导速度所需条件,最终纳入29例患者。传导速度在左房不同结构区域不完全相同(P<0.05);不同电压区域互不相同(P<0.05)。传导速度随左房电压的增长速度可分为三段。当电压<0.8 mV,电压增加1.0 mV,传导速度增加0.28 m/s(P<0.05);当电压处于0.8~1.3 mV,电压每增加1.0 mV,传导速度增加0.53 m/s(P<0.05);当电压>1.3 mV,电压每增加1.0 mV,传导速度增加0.05 m/s(P<0.05)。结论左房传导速度随左房电压的变化呈现出先增加较快后增加减慢的趋势,大致可分为三段,转折点位于左房电压等于0.8mV及1.3mV处。Objective To investigate the dynamic change of left atrial(LA)conduction velocity(CV)with the amplitude of bipolar potential(voltage)in patients with atrial fibrillation(AF).Methods Forty patients were involved in AF catheter ablation.Each LA was artificially divided into six structural regions and four voltage regions according to different regions and voltages.The CV of each regions was calculated respectively.The average CV of each voltage region and the mean value of the corresponding LA voltage were calculated respectively.Results Only 29 patients were included in the final analysis because the others have not meet the standard for calculating CV.CV was not completely the same in different structural regions(P<0.05).CV in different voltage regions were different from each other(P<0.05).CV can be divided into three stages with the increase of LA voltage.When the voltage was less than 0.8 mV,the LA CV increased by 0.28 m/s for every 1.0 mV increase of voltage(P<0.05).When the voltage between 0.8-1.3 mV,the CV increased by 0.53 m/s for every 1.0 mV increase of voltage(P<0.05).When the voltage was>1.3 mV,the CV increased by 0.05 m/s(P<0.05)for every 1.0 mV increase of LA voltage.Conclusion The CV of LA shows more rapid growth at the beginning and slower growth later along with the increase of LA voltage,which can be roughly divided into three sections.The turning point is located where the LA voltage equal to 0.8 mV and 1.3 mV.[Chinese Journal of Cardiac Pacing and Electrophysiology,2020,34(2):128-133]
