摘 要:目的分析脑室出血脑室外引流(EVD)术后颅内感染的危险因素。方法选择80例脑室出血行EVD的患者,根据术后是否发生颅内感染分为感染组18例,非感染组62例,收集两组的临床资料,并进行单因素分析及多因素Logistic回归分析。结果单因素分析显示引流管留置时间>7 d、脑室内注入尿激酶>3次、术后脑脊液渗漏与EVD术后颅内感染有关(P<0.05)。多因素Logistic回归分析发现,脑室引流管的留置时间>7 d和脑脊液渗漏是EVD术后颅内感染的独立危险因素。结论在EVD术后尽量缩短引流管的留置时间、有效处理脑脊液渗漏可以有效地减少EVD术后颅内感染的发生率。Objective To analyze the risk factors of intracranial infection after EVD.Methods 80 cases of EVD patients with ventricular hemorrhage were selected and divided into infected group with 18 cases and non-infected group with 62 cases according to intracranial infection after surgery.Clinical data between the two groups were collected and univariate analysis and multivariate Logistic regression analysis were performed.Results Univariate analysis showed that the indwelling time of the drainage tube was more than 7 days,intraventricular injection of urokinase was more than 3 times,and postoperative cerebrospinal fluid leakage was correlated with intracranial infection after EVD(P<0.05).Multivariate Logistic regression analysis showed that the indwelling time of ventricular drainage tube>7 and cerebrospinal fluid leakage were independent risk factors for intracranial infection after EVD.Conclusion Shortening the indwelling time of drainage tube after EVD and effectively treating CSF leakage can effectively reduce the incidence of intracranial infection after EVD.
