摘　　要：目的评估经尿道前列腺绿激光汽化剜除术和电切术的有效性及安全性,同时更进一步评估其对患者勃起功能的影响。方法回顾性分析2016年1月至2018年12月因前列腺增生症而行经尿道绿激光汽化剜除术(PVRP)和经尿道前列腺电切治疗(TURP)的患者,术前和术后6个月的勃起功能使用国际勃起功能指数(IIEF-5)来测量,同时评估国际前列腺症状评分(IPSS)、生活质量(QoL)、最大尿流率(Qmax)和术中出血量等指标。结果各组IPSS、QoL、Qmax均有显著改善,比较无统计学意义,PVRP的术中出血量更少。PVRP组术后IIEF评分增长(0.74±1.7),而TURP组IIEF-5分数术后减少(0.22±2.0),两种之间的差异具有统计学意义。结论 TURP和PVRP均可有效治疗前列腺增生症,PVRP在术中止血效果更加出色,PVRP更能有效的保留患者的勃起功能。Objective To evaluate efficacy and safety of photoselective vapor-resection of the prostate(PVRP), and investigate the effects of PVRP on erectile function. Methods A retrospective analysis was carried out in the patients who underwent transurethral resection or photoselective vaporization of the prostate for BPH. Their erectile function(EF) was assessed by the International Index of Erectile Function(IIEF-5) before operation and 6 months after operation. International prostate symptom score(IPSS), quality of life(QOL)score, maximum urinary flow rate(Qmax)、intraoperative hemorrhage were also comparatively analyzed between the two groups. Results There were no significant differences in IPSS, QOL,Qmax between the two groups,however,intraoperative hemorrhage of PVRP was lower than that of TURP. Mean IIEF-5 score was increased by 0.74±1.7 in PVRP group, but decreased by 0.22±2.0 in TURP group(P< 0.001). Conclusion Both TURP and PVRP are effective modalities in the management of BPH. PVRP is superior to TURP in intraoperative hemostasis and preserving the erectile function.
