摘  要:目的:评价经阴道超微血管成像(superb microvascular imaging,SMI)技术在早期宫颈癌及癌前病变中的应用价值。方法:随机选取2018-01~11我院诊治的早期宫颈癌及癌前病变患者100例,分为宫颈癌组(Ⅰa~Ⅱa期,n=42)和宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)组(n=58)。另选取同期来我院检查并确认检查结果正常者60例为对照组。采用经阴道超声SMI与彩色多普勒血流显像(color Doppler flow imaging,CDFI)技术检测并比较各组宫颈血流速度,并用Adler半定量血流分析法分析各组血流分级情况。结果:SMI检测发现,宫颈癌组、CIN组宫颈血流速度较对照组低,差异有统计学意义(P<0.05);CDFI检测发现,宫颈癌组宫颈血流速度较CIN组、对照组低,差异有统计学意义(P<0.05)。Adler半定量血流分析结果显示,SMI检测宫颈血流以Ⅱ、Ⅲ级为主,优于CDFI。结论:SMI技术较CDFI能够清晰显示早期宫颈癌及癌前病变中微血管低速血流,可为早期宫颈癌及癌前病变的超声诊断提供一定的应用价值。Objective:To evaluate the value of transvaginal ultrasonography with superb microvascular imaging(SMI)in early-stage cervical cancer and precancerous lesions of uterine cervix.Methods:A total of 100 cases of early-stage cervical cancer and precancerous lesions of uterine cervix were diagnosed and treated in our hospital from January to November 2018,and were randomly divided into cervical cancer group(stageⅠa-Ⅱa,n=42)and cervical intraepithelial neoplasia(CIN)group(n=58).In the same period of time,60 patients who came to our hospital for examination,and the results were normal as control group.The cervical blood flow velocity was compared by transvaginal ultrasound through SMI and CDFI,and Adler semi-quantitative analysis was used to analyze the blood flow grading in each group.Results:The blood flow velocity of cervical cancer group and CIN group were lower than that of control group by SMI,and the differences were statistically significant(P<0.05).The blood flow velocity of cervical cancer group were lower than that of CIN group and control group by CDFI,and the differences were statistically significant(P<0.05).Semi-quantitative analysis of blood flow showed that the overall detection rate of SMI for AdlerⅡandⅢwas significantly higher than that of CDFI.Conclusion:SMI technology is better than CDFI,which can significantly improve the low-velocity blood flow visualization of microvessels in early-stage cervical cancer and precancerous lesions of uterine cervix,and can provide certain application value for ultrasound diagnosis of early-stage cervical cancer and precancerous lesions of uterine cervix.
