摘　　要：目的探讨威伐光治疗仪联合湿润烧伤膏治疗肛门脓肿术后神经痛及伤口恢复的疗效。方法50例肛周脓肿患者随机分为观察组和对照组,所有患者接受一次性的肛周脓肿根治性手术。对照组在术后第2天无菌生理盐水清洗伤口,用凡士林油纱条、无菌纱布换药。观察组于手术后第2天更换敷料后,采用威伐光治疗仪治疗,并使用视觉模拟评分法(VAS)评估疼痛程度。伤口分泌评分、肉芽组织评分、完全衰变时间、伤口愈合时间、伤口减少率、免疫组织化学检测伤口组织中TGF-β1、bFGF、Col-Ⅰ蛋白的表达。结果观察组总有效率明显高于对照组(P<0.05)。术后3周,2组患者的疼痛、伤口分泌物和肉芽组织评分均明显降低(P<0.05),而TGF-β1、b FGF和Col-1则明显升高(P<0.05)。观察组明显优于对照组(P<0.05)。结论湿润烧伤膏换药和威伐光治疗仪的理疗可有效减轻术后肛门脓肿患者疼痛,促进伤口愈合。Objective To investigate the therapeutic effect of Weifaguang therapeutic instrument combined with moist burn ointment on neuralgia and wound recovery after anal abscess.Methods 50 patients with perianal abscess were randomly divided into observation group and control group,all patients received one time perianal Radical abscess surgery.In the control group,the wounds were cleaned with sterile saline on the second day after surgery,and the dressings were replaced with petrolatum gauze and sterile gauze.In the observation group,after the dressing was changed on the second day after the operation,the treatment was carried out with Weivaguang therapeutic apparatus,and the visual analog scale(VAS)was used to assess the degree of pain.The wound secretion score,granulation tissue score,complete decay time,wound healing time,wound reduction rate,and immunohistochemistry were used to detect the expression of TGF-β1,bFGF,and Col I protein in the wound tissue.Results The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).Three weeks after surgery,the pain,wound secretions,and granulation tissue scores of the two groups were significantly reduced(P<0.05),while TGF-β1,b FGF,and Col-1 were significantly increased(P<0.05).The observation group was significantly better than the control group(P<0.05).Conclusion The dressing of MEBO and the physiotherapy of Weifaguang therapeutic instrument can effectively reduce the pain of postoperative anal abscess patients and promote wound healing.
