膀胱及尿道异物的临床特点及治疗策略

摘　　要：目的总结膀胱及尿道异物的临床治疗经验。方法回顾性分析了2000年~2019年我院收治的28例膀胱及尿道异物的临床特点,治疗方法及疗效。共收治28例患者,其中男性19例(青少年3例),女性9例,平均年龄40.3岁,尿道内异物5例,膀胱尿道异物8例,膀胱内异物15例,异物种类繁多,根据不同情况给予了个体化治疗方式,采用内镜下取出异物19例,直接钳夹取出前尿道异物3例,膀胱镜辅助膀胱小切口切开取出异物5例,1例异物自行排出。结果所有患者均取出全部异物,术后随访1~6月,无明显并发症发生。结论治疗不同情况的异物因遵循个体化的手术方式。内镜治疗是膀胱及尿道异物治疗的主要手段,对于不适合用内镜经尿道取出的异物可采用耻骨上膀胱切开或尿道切开取出异物,应同时加强患者的健康教育和心理疏导以减少下尿路异物的发生率。Objectives Toimprove the level of the personalized treatment of urethra or bladder foreign body.Methods The clinical data of 28 cases with foreign bodies in the urethra or bladder in our hospital from 2000 to 2019 were analyzed.There are 19 males(include 3 teenagers)and 9 females,whose average age was 40.5 years.The foreign bodies located at urethra in 5 patients,at urethra and bladder in 8,and at bladder in 15.All patients received personalized treatment according to the type,size,shape,location off oreign bodies,including transurethral endoscopic surgery for 19 cases,direct clamping to remove foreign bodies atanterior urethra for 3 cases.5 cases were performed open surgery.1 case discharged bladder foreign body by himself.Results All foreign bodies were completely removed without significant complications after 1–6 monthfollow-up.Conclusions Transurethral endoscopic surgery is the main treatment way and personalized treatment should be conducted according to the type,size,shape,location of urethra or bladder foreign body to reduce trauma as far as possible.Meanwile,it is important to strengthen the education and psychological counseling for patients to prevent the recurrence of urethra or bladder foreign body.
