健康献血者红细胞血型不规则抗体筛选在临床输血中的意义

摘　　要：目的探究临床输血中健康献血者红细胞血型不规则抗体筛选的重要性。方法于本院2017年3月至2019年5月这一期间,选取12108例健康者的血液标本,所有血液标本都选择微板法对红细胞血型不规则抗体进行筛查,并对不规则抗体检测结果、特异性与效价进行分析与总结。结果12108例健康者血液标本中检出43例不规则抗体,所占比例0.36%;其中男性15例,所占比例0.25%,阳性率0.23%,IgM 13例,所占比例0.22%,IgG 1例,所占比例0.02%,IgM+IgG 0例,所占比例0%;女性28例,所占比例0.11%,阳性率0.13%,IgM 22例,所占比例0.36%,IgG 6例,所占比例0.10%,IgM+IgG 1例,所占比例0.02%;43例不规则抗体分别包括抗-D8例,所占比例18.60%;抗-E 10例,所占比例23.26%;抗-M 9例,所占比例20.93%;冷自体抗体9例,所占比例20.93%;没有确定抗体7例,所占比例16.28%。结论健康献血者予以红细胞血型不规则抗体筛选,可避免溶血反应的出现,对临床输血安全有显著意义。Objective To explore the importance of screening irregular red blood cell blood group antibodies of healthy blood donors in clinical blood transfusion.Methods From March 2017 to May 2019 in our hospital,12108 healthy blood samples were selected.All blood samples were screened for irregular antibody of red blood cell blood type by microplate method,and the detection results,specificity and titer of irregular antibody were analyzed and summarized.Results 43 irregular antibodies were detected in 12108 healthy blood samples,accounting for 0.36%.Among them,there are 15 males,accounting for 0.25%,positive rate 0.23%,IgM 13 cases,accounting for 0.22%,IgG 1 case,accounting for 0.02%,IgM+IgG 0,accounting for 0%;28 women,accounting for 0.11%,positive rate 0.13%,IgM 22 cases,accounting for 0.36%,IgG 6 cases,accounting for 0.10%,IgM+IgG 1 case,accounting for 0.02%;43 cases of irregular antibodies included anti–D 8 cases respectively,accounting for 18.60%;Anti–E 10 cases,accounting for 23.26%;Anti–M 9 cases,accounting for 20.93%;Cold autoantibodies were found in 9 cases(20.93%).No antibody was identified in 7 cases,accounting for 16.28%.Conclusion Screening of irregular antibodies to red blood cell blood type by healthy blood donors can avoid hemolytic reaction and is of significant significance for clinical blood transfusion safety.
