多层螺旋CT对糖皮质激素联合异丙托溴铵治疗慢性阻塞性肺疾病急性加重期的疗效评估

摘　　要：目的:采用多层螺旋CT评估糖皮质激素联合异丙托溴铵治疗慢性阻塞性肺疾病(慢阻肺)的疗效。方法:选择符合纳入标准的60例慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者,将其随机分为对照组与治疗组,每组30例。对照组给予异丙托溴铵治疗,治疗组在对照组的基础上加用糖皮质激素治疗,治疗组及对照组均在治疗开始前及治疗结束后采用多层螺旋CT测定亚段以下水平血管/肺截面面积、亚段水平血管/肺截面面积值。结果:治疗后治疗组与对照组的亚段以下水平血管/肺截面面积、亚段水平血管/肺截面面积值均高于治疗前,差异均有统计学意义(P<0.05);并且治疗组的亚段以下水平血管/肺截面面积、亚段水平血管/肺截面面积值均高于对照组,差异有统计学意义(P<0.05)。结论:采用糖皮质激素联合异丙托溴铵或单独应用异丙托溴铵治疗COPD,均可以减轻患者病情严重程度,具有一定的效果,但糖皮质激素联合异丙托溴铵的效果更佳。Objective Used Multi-slice spiral CT to evaluate the efficacy of glucocorticoids combined with ipratropium bromide in the treatment of chronic obstructive pulmonary disease. Method Sixty patients with chronic obstructive pulmonary disease(COPD) who met the inclusion criteria were selected and randomly divided into control group and treatment group, 30 cases in each group. The control group was treated with ipratropium bromide. The treatment group was treated with glucocorticoid on the basis of the control group. Before treatment and after treatment, both the treatment group and the control group were used multi-slice spiral CT to measure sub-segmental horizontal blood vessels/lung cross-sectional area values,and sub-segment horizontal vascular/lung cross-sectional area values.Results After treatment, the sub-segmental horizontal blood vessels/lung cross-sectional area values and the sub-segment horizontal vascular/lung cross-sectional area values,the control group and the treatment group were higher than those before treatment, the difference was statistically significant(P<0.05);And the sub-segmental horizontal blood vessels/lung cross-sectional area values and the sub-segment horizontal vascular/lung cross-sectional area values of the treatment group were higher than the control group,the difference was statistically significant(P<0.05).Conclusion The treatment of COPD with glucocorticoid combined with ipratropium bromide or ipratropium bromide alone can alleviate the severity of the disease and has a certain effect, but the effect of glucocorticoid combined with ipratropium bromide is better.
