超声造影在腹壁下动脉穿支皮瓣乳房再造术中的应用

摘　　要：目的:应用超声造影增强技术对腹壁下动脉穿支皮瓣血管检测及评估,为临床提供直观形象的术前影像资料。方法:在常规高频彩超基础上,应用超声造影增强技术对25例预行乳房再造术患者进行穿支血管评估筛选。观察穿支血管的走行、与主干血管的关系及其在皮瓣内的分支、分布情况;并标记穿支血管穿出点的位置。结果:超声造影增强技术清晰地显示了25例患者腹壁下动脉的穿支血管,较为粗大的穿支血管数量每侧约有1~3支,于腹部两侧基本呈对称性分布。这些穿支血管主要分布于脐下8cm范围内,管径为(0.68±0.33)mm,血流峰值流速为10~36 cm/s,阻力指数为0.6~0.88。所有病例均在术中证实超声所见,术后皮瓣存活率100%。结论:超声造影能更清晰地显示和定位腹壁下动脉穿支血管,并为术前及术中提供更准确的参考依据。在皮瓣血管检查中发挥着重要的作用。Objective To detect and evaluate the vessels of perforator flap of inferior abdominal artery by contrast-enhanced ultrasound,so as to provide visual image data for clinic.Method On the basis of conventional high-frequency ultrasound,25 cases of breast reconstruction were evaluated by contrast-enhanced ultrasound.The course of the perforator,the relationship between the perforator and the main vessel,the branches and distribution in the flap were observed,and the location of the perforator point was marked.Results Contrast-enhanced ultrasound clearly showed the perforating vessels of the inferior abdominal artery in 25 patients.The number of the larger perforating vessels was about 1~3 on each side,which was basically symmetrical on both sides of the abdomen.These perforating vessels were mainly distributed in the area of 8 cm below the umbilicus,with a diameter of(0.68 ± 0.33) mm,a peak flow velocity of 10~36 cm/s and a resistance index of 0.6~0.88.All cases were confirmed by ultrasound during operation,and the survival rate of flap was 100%.Conclusion Contrast-enhanced ultrasound can show and locate the perforating vessels of inferior abdominal artery more clearly,and provide more accurate reference for preoperative and intraoperative.It plays an important role in vascular examination of flap.
