摘 要:目的探讨2型糖尿病(type 2 diabetes mellitus,T2DM)患者血清癌胚抗原(carcinoembryonic antigen,CEA)、血清鳞状细胞癌抗原(squamous cell carcinoma antigen,SCC)、细胞角蛋白19片断(CYFRA21-1)水平及与尿N-乙酰-β-D-氨基葡萄糖苷酶(n-acetyl-β-d-glucosaminidase,NAG)的关系。方法收集140例T2DM例患者为患者组,根据NAG水平将T2DM患者分为两组,即T2DM合并肾小管损伤组(n=70)和单纯T2DM组(n=70),另以83例健康体检者作为正常对照组,分别检测CEA、SCC、CYFRA21-1、尿NAG,另收集空腹血糖、糖化血红蛋白、尿蛋白/肌酐比、糖尿病病程等资料。结果T2DM合并肾小管损伤组的CEA、SCC、CYFRA21-1、尿NAG与单纯T2DM组、正常对照组差异均有统计学意义(P<0.05)。相关性分析显示,CYFRA21-1(r=0.627)、SCC(r=0.693)、CEA(r=0.324)水平均与NAG呈显著正相关。Logistic回归分析结果显示,NAG是T2DM患者CYFRA21-1和SCC水平升高的危险因素(OR值分别为2.51与1.13,P<0.01)。结论T2DM患者合并肾小管损伤组肿瘤标志物CEA、SCC、CYFRA21-1较正常组和单纯DM组均有升高,其中SCC、CYFRA21-1水平与肾小管功能受损有密切关系,临床判断时应结合尿NAG水平综合考虑。Objective To investigate the association between carcinoembryonic antigen(CEA),squamous cell carcinoma antigen(SCC),cytokeratin 19 fragment(CYFRA21-1)and urinaryβ-N-acetylgucosaminidase(NAG)in patients with type 2 diabetes mellitus(T2DM).Methods 140 T2DM patients were collected as the patient group.These patients were divided into 2 groups according to NAG levels:the DM with tubular lesion group(n=70)and the DM group(n=70).At the same time,a normal control group(NC group)with 83 cases of healthy check-up was formed.The levels of serum CEA,SCC,CYFRA21-1 and urinary NAG were detected among all subjects.We also collected information on fasting plasma glucose,HbA1c,urinary albumin-to-creatinine ratio and diabetic duration.Results CEA,SCC and CYFRA21-1 levels were significantly increased in the group of T2DM with tubular lesion than in the DM group and NC group(P<0.05).CYFRA21-1(r=0.627),SCC(r=0.693),and CEA(r=0.324)were positively and significantly associated with NAG in T2DM patients(r=0.324,0.693 and 0.627,respectively).Logistic regression analysis showed that NAG was an independent risk factor for elevated serum CYFRA21-1 and SCC levels(OR=2.51 and 1.13,respectively)in T2DM patients.Conclusion CEA,SCC and CYFRA21-1 levels are significantly higher in T2DM patients with tubular lesion,and the elevation of SCC and CYFRA21-1 levels are closely associated with renal tubule lesions.NAG level should be taken into the consideration for the clinical interpretation to avoid potential misdiagnoses.
