摘　　要：目的探讨前置胎盘附着子宫瘢痕部位妊娠对母婴结局的影响。方法将84例前置胎盘瘢痕子宫妊娠产妇按胎盘附着子宫部位分为2组:观察组42例前置胎盘附着于子宫瘢痕处,对照组42例前置胎盘未附着于子宫瘢痕处。比较2组孕产史(包括剖宫产次数、流产史等)、产前出血情况、胎盘情况(包括胎盘位置、前置胎盘类型等)、妊娠结局(包括产后出血、输血、胎盘植入、子宫切除、早产情况、紧急剖宫产发生率等)、术中出血量、术后住院时间、新生儿体重情况。结果2组剖宫产次数、流产史、产前出血、胎盘位置、前置胎盘类型、紧急剖宫产发生率、新生儿体重比较差异均无统计学意义(P>0.05);观察组产后出血、输血、胎盘植入、子宫切除、早产等发生率及术中出血量、术后住院时间均较对照组显著升高(P<0.05)。结论前置胎盘附着子宫瘢痕部位对妊娠结局有一定的影响,会影响到产后出血、输血、胎盘植入、子宫切除、早产情况、术中出血量、术后住院时间,临床需引起足够的重视。Objective To investigate the effect of placental attachment to uterine scar on pregnancy outcomes in women with placenta previa.Methods A total of 84 parturient women with placenta previa in scarred uterus were divided into two groups:observation group(placenta was attached to uterine scar)and control group(placenta was not attached to uterine scar),with 42 women in each group.Pregnancy history,antepartum hemorrhage,placental condition,pregnancy outcomes,intraoperative blood loss,postoperative hospital stay and neonatal body mass were compared between the two groups.Results There were no significant differences between the two groups in cesarean section frequency,abortion history,prenatal bleeding,placenta position,placenta previa type,incidence of emergency cesarean section and neonatal body mass(P>0.05).Compared with control group,the incidence of postpartum hemorrhage,blood transfusion,placental implantation,hysterectomy and premature delivery,amount of intraoperative bleeding and length of postoperative hospital stay significantly increased in observation group(P<0.05).ConclusionThe site of placental attachment in scarred uterus can affect postpartum hemorrhage,blood transfusion,placental implantation,hysterectomy,premature delivery,intraoperative blood loss and postoperative hospital stay in women with placenta previa.
