大骨节病和踝关节骨关节炎患者血清IL-1β、IGF-1水平与VAS评分的关系

摘　　要：目的 观察大骨节病(KBD)和踝关节骨关节炎(OA)患者血清和关节滑液中白细胞介素(IL)-1β、胰岛素样生长因子(IGF)-1表达水平,评价IL-1β、IGF-1水平与视觉模拟量表(VAS)评分间的关系.方法 采用成组设计,分别以2010年1月至2016年12月在陕西省人民医院骨科住院拟进行踝关节镜清理术的KBD、OA患者各20例为KBD、OA组,同时以无距骨损伤的踝关节骨折患者20例作为对照组.采集所有研究对象的关节滑液及外周血,采用酶联免疫吸附法检测IL-1β、IGF-1表达水平;采用美国国立卫生研究所制定的VAS对患者术前及术后3、7、14、28 d疼痛程度进行测量,采用相关性分析评估IL-1β、IGF-1水平与VAS评分的关系.结果 KBD和OA患者关节滑液IL-1β[(67.32±6.22)、(56.46±5.43)pg/ml]、IGF-1[(24.36±6.22)、(21.45±4.35)pg/ml]水平显著高于对照组[(27.01±3.15)、(10.21±2.50)pg/ml,P均<0.05].KBD和OA患者血清IL-1β、IGF-1水平显著高于对照组(P均< 0.05).KBD和OA患者术前及术后3、7、14、28 d血清IL-1β、IGF-1水平与VAS评分呈正相关(r=0.427、0.502,0.562、0.628,P均<0.05).结论 KBD与OA患者关节滑液和血清IL-1β及IGF-1水平明显升高,血清中IL-1β及IGF-1水平与VAS评分呈正相关.Objective To detect the expression levels of interleukin(IL)-1β,insulin-like growth factors(IGF)-1 in serum and synovial fluid of patients with Kaschin-Beck disease(KBD)and osteoarthritis(OA)and analyze the relationship between IL-1β,IGF-1 levels and visual analogue scale(VAS)score.Methods From January 2010 to December 2016,60 patients visited the department of orthopedics of Shaanxi Provincial People's Hospital were selected,included 20 patients with KBD(arthroscopic debridement of ankle),20 patients with OA(arthroscopic debridement of ankle),and 20 patients with ankle fracture(no talus injury)as control group.The peripheral blood and synovial fluid samples from the patients were collected.Enzyme-linked immunosorbent assay was used to detect the expression levels of IL-1β,IGF-1 in serum and synovial fluid.In each group,VAS score was calculated pre-operative and post-operative at 3,7,14 and 28 d.The relationship between IL-1β,IGF-1 levels and VAS score was evaluated by correlation analysis.Results Synovial fluid IL-1βand IGF-1 levels in the KBD and OA groups were(67.32±6.22),(56.46±5.43)and(24.36±6.22),(21.45±4.35)pg/ml,respectively,which were higher than those in the control group[(27.01±3.15),(10.21±2.50)pg/ml,P<0.05].Meanwhile,serum IL-1βand IGF-1 levels in the KBD and OA groups were higher than those in the control group(P<0.05).Pearson correlation analysis showed that the expression levels of IL-1βand IGF-1 in serum pre-operative and post-operative at 3,7,14 and 28 d were positively correlated with VAS score in KBD and OA groups,respectively(r=0.427,0.502,0.562,0.628,P<0.05).Conclusion The levels of IL-1βand IGF-1 in synovial fluid from ankle joints and serum are significantly increased in KBD group and OA group,and the levels of IL-1βand IGF-1 in serum are positively correlated with VAS score,respectively.
