低出生体重早产儿1岁内体格追赶生长的纵向研究

摘　　要：目的研究低出生体重早产儿1岁内体格生长特点,以促进早产儿适宜的体格追赶。方法 2016年10月至2019年10月在西安市第四医院儿童保健中心建立专案管理,并进行规律随访的292例低出生体重早产儿,其中小于胎龄儿(SGA)早产儿103例,适于胎龄儿(AGA)早产儿189例,同期随机抽取355例足月儿作为对照组,并在(矫正)40周、3月龄、6月龄、9月龄、12月龄时进行体重、身长、头围的测量,计算Z评分,采用重复测量资料的方差分析,评估早产儿体格发育特点。结果 (矫正)40周、3、6、9、12月龄早产儿SGA组、AGA组和足月儿组之间体重、身长和头围Z评分均存在统计学差异(均P<0.05);AGA和SGA早产儿在矫正40周~6月龄体重追赶趋势明显,AGA早产儿在矫正40周~3月龄身长和头围追赶趋势明显,SGA早产儿身长在矫正6~12月龄、头围在矫正3~12月龄追赶趋势明显;三组的体重、身长、头围生长速率在矫正40周~3月龄期间均存在统计学差异(F值分别为5.295、4.047、3.421,均P<0.05),且AGA相对较快,3~6月龄头围生长速率三组之间存在统计学差异(F=4.559,P<0.05),且SGA组相对较快。结论 1岁内低出生体重早产儿SGA和AGA组生长追赶趋势不同,AGA早产儿追赶生长主要在6月龄内,SGA早产儿主要在12月龄内。SGA早产儿体格发育水平各时间点均落后于AGA早产儿和足月儿。Objective To study the physical growth characteristics of low birth weight premature infants within 1 year old,in order to promote proper physical catch-up growth in preterm infants.Methods From October 2016 to October 2019,292 cases of low birth weight premature infants,103 of whom were premature infants with small gestational age(SGA),and 189 of whom were appropriate gestational age(AGA)premature infants.All cases were built project management and followed up regularly in the Children’s Health Center of Xi’an Fourth Hospital.355 term infants were randomly selected as the control group at the same time.The weight,length and head circumference were measured at 40 weeks,3 months,6 months,9 months and 12 months of corrected age.The Z-score was calculated and the analysis of variance of repeated measurement data was used to evaluate the physical growth characteristics of premature infants.Results There were statistically significant differences in Z scores of weight,length and head circumference between SGA preterm infants,AGA preterm infants and term infants at 40 weeks,3,6,9 and 12 months of corrected age(all P<0.05).The weight of SGA and AGA premature infants at 40 weeks-6 months of corrected age showed a catch-up trend,the length and head circumference of AGA premature infants at 40 weeks-3 months of corrected age showed a catch-up trend,the length of SGA premature infants at 6-12 months of corrected age showed a catch-up trend,and the head circumference of SGA premature infants at 3-12 months of corrected age showed a catch-up trend.There were significant differences in weight,length and head circumference growth rate at 40 weeks-3 months of corrected age among the three groups(F=5.295,4.047,3.421,all P<0.05),and the AGA was relatively fast.There were statistical difference in the growth rate of head circumference at 3-6 months of corrected age among the three groups(F=4.559,P<0.05),and the SGA was relatively fast.Conclusion The growth catch-up trend of SGA and AGA group were different in low birth weight p
