摘  要:目的探讨孕妇孕期营养状况与婴儿免疫应答水平的关联。方法将2017年1月1日至8月31日,在浙江大学医学院附属妇产科医院门诊规范建档进行定期产检的130例孕妇,根据孕期营养干预方法不同分为干预组(n=80)和对照组(n=50),对照组孕妇给予常规营养指导,干预组孕妇实施有针对性的孕期营养干预。分析和统计两组孕妇孕期营养状况及三大营养素摄入量所占热量百分比,并于分娩后28d采用流式细胞仪测定两组婴儿免疫应答指标,包括CD19^+、CD56^+B淋巴细胞及免疫球蛋白(IgG、IgM、IgA)、C3、C4。结果以供给量标准为依据,干预组孕妇蛋白质、膳食纤维、维生素B1、钙、铁、锌摄入量明显高于对照组(t值分别为4.889、6.744、6.563、4.299、4.479、6.531,均P<0.05),干预组脂肪、糖摄入量明显低于对照组(t值分别为4.988、3.802,均P<0.05);干预组孕妇孕期三大营养素所占热量百分比,其中碳水化合物、蛋白质比例高于对照组,脂肪比例低于对照组(t值分别为2.642、3.270、2.865,均P<0.05)。与对照组婴儿相比,干预组婴儿CD19^+、CD56^+水平明显高于对照组(t值分别为2.642、3.270,均P<0.05),干预组婴儿IgG、IgM、IgA、C3、C4水平明显低于对照组(t值分别为2.865、4.393、2.281、3.354、3.344,均P<0.05)。结论对孕妇实施有针对性的孕期营养干预有助于优化和平衡孕期营养,改善婴儿免疫应答水平;孕期营养状况与婴儿免疫应答水平具有关联性。Objective To explore relationship between maternal nutritional status during pregnancy and her infant’s immune response.Methods 130 pregnant women who received regular prenatal examination in Gynecological and Obstetric Clinic of The Affiliated Obstetrical and Gynecological Hospital,School of Medicine,Zhejiang University from January 1 st,2017 to August 31 st,2017 were divided into intervention group(n=80) and control group(n=50) according to their nutritional intervention during pregnancy.The pregnant women in the control group were given routine nutritional guidance.While those in the intervention group were given targeted nutrition intervention during pregnancy based on National Reference Allowance of Nutrients from Daily Dietary of Chinese People.The nutritional status of the pregnant women in the two groups and percentages of calories produced by three major nutrients were analyzed.The infantile immune response indexes in the two groups including B lymphocytes CD19+ and CD56+,immunoglobulins IgG,IgM and IgA,complements C3 and C4 were detected by flow cytometry at the 28 th day after delivery.Results The intakes of protein,dietary fiber,vitamin B1,calcium,iron and zinc of the pregnant women in the intervention group were significantly higher than those of the pregnant women in the control group(protein:88.30±12.83 vs.77.65±10.77;dietary fiber:17.27±3.28 vs. 13.36±3.11;vitamin B1:1.63±0.29 vs. 1.30±0.26;calcium:998.26±44.87 vs. 951.32±79.59;iron:25.56±5.22 vs. 21.43±4.94 and zinc:14.26±2.37 vs. 11.31±2.71)(t=4.889,6.744,6.563,4.299,4.479 and 6.531 respectively,all P<0.05),while the intakes of fat(70.49±10.60) and carbohydrate(261.64±19.49) of the pregnant women in the intervention group were significantly lower than those of the pregnant women in the control group(fat:79.71±9.61;sugar:275.80±22.42)(t=4.988 and 3.802 respectively,both P<0.05).In addition,in the three major nutrients,the percentages of calories from carbohydrate(13.48±2.69) and protein(32.82±4.59) of the pregnant women in the
