真实世界中药降压宝系列治疗原发性高血压降低靶器官损害的评价

摘　　要：目的观察中药降压宝系列治疗中老年原发性高血压靶器官损害的临床疗效。方法采用回顾性队列研究对1 000例中老年高血压患者的临床观测数据进行倾向评分匹配研究,以服用中药累计时间42个月为节点分为强暴露组345例和弱暴露组619例,对服药时间长短对靶器官损害(左心室肥厚、颈动脉斑块、肌酐轻度升高)情况进行分析。结果弱暴露组易发生靶器官损害(HR=1.27,P=0.00),倾向评分各种统计方法结果一致。靶器官损害中左心室肥厚、颈动脉斑块有显著的统计学差异(P<0.05)。结论中药降压宝在协调治疗高血压方面有保护靶器官,减少靶器官损害的作用。倾向评分法可以有效地均衡混杂因素,为中医药临床研究提供有效的新途径。Objective To observe the clinical effect of Jiangyabao series of traditional Chinese medicine on target organ damage in elderly patients with essential hypertension.Methods A retrospective cohort study was conducted to match the clinical observation data of 1 000 middle-aged and elderly patients with hypertension in Henan province from January 2000 to December 2010. The patients were divided into strong exposure group(n=345) and weak exposure group(n=619) based on the 42-month cumulative time of taking traditional Chinese medicine. The damage of target organs(left ventricular hypertrophy, carotid artery plaque, and mild creatinine increasing) was analyzed.Results Weak exposure group was prone to target organ damage, with significant statistical difference(HR=1.27, P=0.00). The results of tendency score were consistent with those of various statistical methods. There were significant statistical differences in left ventricular hypertrophy and carotid plaque in target organ damage(P<0.05).Conclusions Jiangyabao could protect target organs and reduce damage of target organs in the coordinated treatment of hypertension. Tendency score method could effectively balance the confounding factors and provide an effective new way for clinical research of traditional Chinese medicine.
