摘 要:目的建立脑缺血大鼠模型,给予其十二井穴刺络放血治疗,观察十二井穴刺络放血对大鼠脑组织缺氧状态及能量代谢紊乱的影响。方法选购50只清洁大鼠,在其左右两侧纹状体实施微透析套管埋植术,术后随机抽取10只大鼠作为空白对照组,其他大鼠进行大脑中动脉闭塞(MCAO)模型复制,取造模成功的33只大鼠随机分为3组,包括模型组、吸氧组、十二井穴刺络组,各11只。模型组不给予干预,吸氧组每天给予50%浓度氧气吸入60 min。十二井穴刺络组每天给予十二井穴刺络放血治疗。连续治疗3 d后,检测并比较各组大鼠脑组织[一氧化氮合酶(NOS)、肿瘤坏死因子(TNF)-α、组织型纤溶酶原激活物(tPA)]因子水平;检测并比较各组大鼠双侧纹状体细胞外液γ-氨基丁酸(GABA)、谷氨酸(Glu)含量。结果空白组脑组织中NOS、TNF-α水平表达最低,tPA水平最高,后依次为十二井穴刺络组、吸氧组、模型组,除十二井穴刺络组NOS指标与空白组无显著差异外,组间比较差异均有统计学意义(P<0.05);空白组GABA、Glu含量最低,后依次为十二井穴刺络组、吸氧组、模型组,组间比较差异均有统计学意义(P<0.05)。结论十二井穴刺络放血能够减轻脑缺血大鼠一氧化氮神经毒性,改善大脑缺氧状态,纠正大脑能量代谢紊乱的情况,调节大鼠脑组织微循环状态,保护脑神经细胞,对阻止脑缺血大鼠脑损伤进一步发展有着积极意义。Objective To establish the rat model with cerebral ischemia, to provide twelve well-points blood-letting puncture therapy, and to observe the effect of twelve well-points blood-letting puncture on anaerobic condition and energy metabolism disorder in rats.Methods 50 clean rats were purchased, and the microdialysis cannula implantation was implemented on the left and right striatum. After operation, 10 rats were randomly selected as blank control group, and other rats were made MCAO model. 33 rats with successful modeling were randomly divided into three groups, including model, oxygen-inhalation and twelve well-points puncture groups, with 11 rats in each group. Model group was not given intervention, oxygen-inhalation group was given 50% oxygen uptake per day for 60 min, and twelve well-points puncture group was given twelve well-points blood-letting puncture every day. After treatment for 3 days, the brain tissues [nitric oxide synthase(NOS), tumor necrosis factor(TNF)-α, tissue-type plasminogen activator(tPA)] of rats in each group were detected and compared. The contents of γ-aminobutyric acid(GABA) and glutamic acid(Glu) in the extracellular fluid of bilateral striatum of rats in each group were detected and compared.Results The expressions of NOS and TNF-α in the brain tissues in blank group were the lowest, followed by twelve well-points puncture, oxygen-inhalation and model groups, except that there was no statistical difference in the NOS between twelve well-points puncture and blank groups, the differences between the groups were statistically significant(P<0.05);the contents of GABA and Glu in blank group were the lowest, followed by twelve well-points puncture, oxygen-inhalation and model group, and the differences between the groups were statistically significant(P<0.05).Conclusions Twelve well-points blood-letting puncture for rats with cerebral ischemia could alleviate nitric oxide neurotoxicity, improve cerebral anaerobic condition, correct cerebral energy metabolism disorder, regulate microcir
