摘  要:目的探讨胎膜早破(PROM)胎膜组织核苷酸结合寡聚化结构域受体1(NOD1)及沉默配型信息调节蛋白1(SIRT1)的表达与羊水中白细胞介素(IL)-1β、基质金属蛋白酶3(MMP-3)水平的相关性,为深入了解胎膜早破的发病机制并寻找可能的监测及治疗方法提供指导。方法90例妊娠妇女分为3组:足月胎膜早破(tPROM)组、未足月胎膜早破(pPROM)组及正常足月妊娠(正常对照)组,每组各30例。胎膜组织NOD1与SIRT1表达部位及含量测定采用免疫组织化学法及Western blot,羊水中IL-1β、MMP-3水平检测采用ELISA方法,分析NOD1、SIRT1与羊水IL-1β及MMP-3表达的相关性。结果PROM组NOD1、IL-1β及MMP-3表达高于正常对照组(P<0.05),SIRT1表达低于正常对照组(P<0.05)。pPROM组NOD1、IL-1β、MMP-3表达高于tPROM组(P<0.05),tPROM组与pPROM组SIRT1表达差异无统计学意义(P>0.05)。胎膜组织NOD1表达与羊水IL-1β及MMP-3水平呈正相关(P<0.01);胎膜组织SIRT1表达与羊水IL-1β及MMP-3水平呈负相关(P<0.01)。结论胎膜组织中NOD1及羊水中IL-1β及MMP-3表达水平升高、胎膜组织中SIRT1表达水平降低与胎膜早破发生相关,且4种因子在胎膜早破发病中存在相关性。ObjectiveTo explore the expressions of nucleotide-binding oligomerization domain-containing 1(NOD1)and silent information regulator 1(SIRT1)in the fetal membranes and to investigate their relationship with matrix metalloproteinase-3(MMP-3)and IL-1βlevels in amniotic fluid from women with premature rupture of the membranes(PROM),and to provide guidance for further understanding the pathogenesis and treatment of PROM.MethodsA total of90 pregnant women were separated into three groups:term premature rupture of the membranes(tPROM)group,preterm premature rupture of the membranes(pPROM)group and healthy control group.The location and expression levels of NOD1 and SIRT1 in the fetal membranes were measured by immunohistochemistry and Western blot assay.IL-1βand MMP-3 levels in amniotic fluid were detected with ELISA.The relationship between the expressions of NOD1,SIRT1,IL-1βand MMP-3 was assessed.Results The expressions of NOD1,IL-1βand MMP-3 were significantly higher in PROM group compared with those of healthy control group,but the expression of SIRT1 was lower than that of healthy control group(P<0.05).Furthermore,the expressions of NOD1,IL-1βand MMP-3 were significantly higher in pPROM group than those in tPROM group(P<0.05).There was no significant difference in the SIRT1 expression between tPROM group and pPROM group(P>0.05).The expression of NOD1 was positively correlated with the levels of IL-1βand MMP-3(P<0.01).The expression of SIRT1 was negatively correlated with the levels of IL-1βand MMP-3(P<0.01).ConclusionThe level of NOD1 in fetal membranes and levels of IL-1βand MMP-3 in amniotic fluid are increased.The level of SIRT1 in fetal membranes is decreased.Changes of the expressions of the four proteins are correlative and related with the occurrence of PROM.
