摘  要:目的:探讨灯盏花素对大鼠冠状动脉微栓塞(CME)后心肌炎症反应的影响及意义。方法:将30只SD大鼠随机地分为假手术组、CME组和CME+灯盏花素组,每组10只。CME组和CME+灯盏花素组经左心室注射直径为42μm的微栓塞球建立CME大鼠模型,假手术组用等量生理盐水代替,CME+灯盏花素组在CME造模前7 d连续给予灯盏花素40 mg/kg灌胃,1次/d。术后12 h,分别用心脏超声评估大鼠心功能,苏木精-伊红(HE)染色观察心肌组织病理学改变,实时荧光定量PCR(qPCR)及蛋白质免疫印迹(Western blotting)检测心肌组织促炎症介质肿瘤坏死因子α(TNF-α)及白细胞介素1β(IL-1β)的表达水平。结果:与假手术组相比,CME组左心室舒张末期内径(LVEDd)及左心室收缩末期内径(LVESd)显著增长,左心室射血分数(LVEF)及左心室短轴缩短率(LVFS)显著下降(P<0.05);心肌组织TNF-α和IL-1β的mRNA及蛋白表达水平显著升高(P<0.05)。与CME组相比,CME+灯盏花素组的LVEDd及LVESd显著减短,LVEF及LVFS显著升高(P<0.05);心肌组织TNF-α和IL-1β的mRNA及蛋白表达水平显著降低(P<0.05)。结论:灯盏花素预处理可明显减轻CME后心肌炎症反应并改善心脏功能障碍,其机制可能是通过降低心肌组织中TNF-α和IL-1β的表达水平来实现。Objective:To explore the effect and significance of breviscapine on myocardial inflammation after coronary artery microembolization(CME)in rats.Methods:Thirty SD rats were randomly divided into sham operation group,CME group,and CME+breviscapine group,with 10 rats in each group.The CME rat model was established by injecting a diameter 42μm microembolic spheres into the left ventricle of CME group and CME+breviscapine group.The sham operation group was injectedwiththe same amount of normal saline.CME+breviscapine group was given 40 mg/kg of breviscapineby intragastricadministrationonce a dayfor 7 days before CME modeling.After 12 hours of operation,cardiac function was evaluated by echocardiography,myocardial histopathological changes were observed by HE staining,and the expressions of proinflammatory mediators tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β)of myocardial tissuewere detected by real-time fluorescence quantitative PCR(qPCR)and Western blotting.Results:In CME group,left ventricular enddiastolic diameter(LVEDd)and left ventricular endsystolic diameter(LVESd)were significantly increased,left ventricular ejection fraction(LVEF)and left ventricular fractional shortening(LVFS)were significantly decreased(P<0.05),and the mRNA and protein expression of TNF-αand IL-1βof myocardial tissue were significantly increased(P<0.05)compared with shamoperation group.In CME+breviscapine group,LVEDd and LVESd were significantly decreased,LVEF and LVFS were significantly increased(P<0.05),mRNA and protein expression of TNF-αand IL-1βin myocardial tissue were significantly decreased(P<0.05)compared with CME group.Conclusion:Breviscapine preconditioning can significantly reduce myocardial inflammatory response and improve cardiac dysfunction after CME,which may be achieved by reducing the expression of TNF-αand IL-1βin myocardial tissue.
