摘 要:目的:建立广西南宁地区妊娠期妇女孕8周前特异性促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)的正常参考值范围及探讨血清铁蛋白(SF)、25-羟维生素D水平对甲状腺功能的影响。方法:采用化学发光技术测定南宁地区180例单胎妊娠健康孕8周前妇女的FT3、FT4及TSH水平;采用全自动免疫分析仪测定SF及25-羟基维生素D水平。结果:正常健康妊娠期妇女孕8周前FT3范围为(3.47~5.80)pmol/L,FT4为(7.51~13.88)pmol/L,TSH为(0.14~4.27)mIU/L。SF为(72.93±49.94)ng/mL,25-羟基维生素D为(38.98±10.67)nmol/L。不同SF及25-羟基维生素D水平下甲状腺相关激素TSH、FT3、FT4水平比较,差异无统计学意义(P>0.05)。结论:建立了广西南宁地区孕8周前特异性TSH、FT3、FT4的正常参考范围。本地区孕8周前孕妇的SF水平整体处于正常水平,25-羟基维生素D水平整体处于维生素D缺乏状态,未发现孕早期铁缺乏和维生素D缺乏对孕早期甲状腺功能产生影响。Objective:To establish the reference range for specific thyroid stimulating hormone(TSH),free triiodothyronine(FT3),and free thyroxine(FT4)in pregnant women in Nanning.Toexplore the effect of serumferritin(SF)and 25-hydroxyvitamin D levels on thyroid function.Methods:The chemiluminescence technique was used to determine the FT3,FT4 and TSH levels of 180 healthy single-pregnancy women with less than 8 weeks of gestation in Nanning.The SF and 25-hydroxyvitamin D levels were measured by an automatic immunoassay analyzer.Results:In normal healthy pregnant women withless than 8 weeks of gestation,the level of FT3 wasranged from 3.47 to 5.80 pmol/L,FT4 wasfrom 7.51 to 13.88 pmol/L,and TSH wasfrom 0.14 to 4.27 mIU/L,and SF was(72.93±49.94)ng/mL,25-hydroxyvitamin D was(38.98±10.67)nmol/L.The levels of thyroid-related hormonesTSH,FT3 and FT4-at different levels of SF and 25-hydroxyvitamin D showed no significant difference(P>0.05).Conclusion:The reference range for specific TSH,FT3 and FT4 insingle-pregnancy women with less than8 weeks of gestation in Nanning,Guangxi is established.The SF level of pregnant women with less than 8 weeks of gestation in this area is generally at a normal level,and the level of 25-hydroxyvitamin D is generally in a state of vitamin D deficiency.No iron deficiency and vitamin D deficiency in early pregnancy have been found to have an effect on thyroid function in early pregnancy.
