益生菌联合针灸在肝硬化自发性腹膜炎治疗中的疗效及对凝血功能的影响

摘　　要：目的:探讨益生菌联合针灸在肝硬化自发性腹膜炎治疗中的应用价值及对凝血功能的影响。方法:选取2015年1月-2017年12月甘肃武威肿瘤医院收治的肝硬化并发自发性细菌性腹膜炎(Spontaneous Bacterial Peritonitis,SBP)的患者141例作为研究对象,按随机数字表法分为空白组、对照组、试验组,其中空白组48例,对照组46例,试验组47例。空白组予以常规抗感染治疗;对照组在空白组基础上给予口服双歧杆菌三联活菌肠溶胶囊;试验组在对照组基础上给予温针足三里穴,电针支沟、三阴交、太溪穴,连续治疗10天。观察三组临床治疗效果;对比静脉血中C-反应蛋白、白介素-6、降钙素的转归;对比静脉血中血小板、凝血功能的变化。结果:经10天治疗后,试验组总有效率91.5%(43/47)高于空白组66.7%(32/48)及对照组78.3%(36/46)(P<0.05);与对照组及空白组相比,试验组患者静脉血中相关炎性指标改善明显(P<0.05);血小板及凝血功能与治疗前相比,试验组显著优于空白组(P<0.01)。结论:双歧杆菌三联活菌肠溶胶囊联合针灸在肝硬化自发性腹膜炎的治疗中有提高疗效、缩短病程的积极意义,并能改善凝血功能,降低患者出血风险。Objective:To explore the value of probiotics combined with acupuncture in the treatment of spontaneous peritonitis of cirrhosis(SBP)and its effect on coagulation function.Methods:141 patients with liver cirrhosis complicated with SBP from January 2015 to December 2017 were selected as the research subjects.They were divided into the blank group,the control group and the experience group randomly.There were 48 patients in the blank group,46 patients in the control group and 47 patients in the experiment group.The blank group was given routine anti-infection treatment.The control group was given Bifid,Triple Viable Capsule Dissolving at Intestines on the basis of the anti-infection treatment.The experimental group was given warm needling Zusanli(足三里)acupoint,electroacupuncture at Zhigou(支沟),Sanyinjiao(三阴交)and Taixi(太溪)acupoints for 10 days on the basis of the treatment of the control group.The clinical effect of the three groups,the change of C-reactive protein,interleukin-6 and calcitonin in venous blood,the influence of platelet and coagulation in venous blood of three groups were observed and compared.Results:After 10 days of treatment,the total effective rate of the experimental group 91.5%(43/47)was higher than that of the blank group 66.7%(32/48)and the control group 78.3%(36/46)(P<0.05).Compared with the control group and the blank group,the related inflammatory indexes in the venous blood of the experimental group were improved significantly(P<0.05),and the platelet and coagulation function in the experimental group were significantly better than those in the blank group before treatment(P<0.01).Conclusion Bifid.Triple Viable Capsule Dissolving at Intestines combined with acupuncture has positive significance in improving the curative effect,shortening the course of disease,improving coagulation function and reducing the risk of bleeding in the treatment of spontaneous peritonitis of liver cirrhosis.
