摘 要:目的探讨1HLA-DRB1基因多态性在早发型重度子痫前期中的表达及对妊娠结局的影响。方法选取2018年2月-2019年2月我院收治的早发型重度子痫前期患者60例作为研究组,同时选取同期在我院进行体检的健康孕妇60例作为对照组。对两组人群人类白细胞抗原基因DRB1(1HLA-DRB1基因多态性)进行检测,比较两组产妇、新生儿结局、对照组孕妇与新生儿1HLA-DRB1基因相关SNP基因型分布情况、孕妇1HLA-DRB1基因相关SNP基因型分布情况。结果研究组年龄、舒张压、收缩压、生产次数高于对照组(P<0.05),研究组分娩孕周小于对照组(P<0.05),研究组怀孕次数与对照组相比,无明显差异(P>0.05);研究组新生儿出生体重、胎龄、5min、1minApgar评分小于对照组(P<0.05),研究组新生儿宫内窘迫、窒息发生率高于对照组(P<0.05);此次研究在2个基因片段中发现3个SNP点,为rs114293611、rs2213586、rs3135388位点,rs2308765在研究中未表现出多态性;研究组rs114293611基因型分布与对照组相比,存在一定的差异(P<0.05),两组孕妇1HLA-DRB1基因的rs2213586、rs3135388位点基因型分布,无明显差异(P>0.05)。结论早发型重度子痫前期中,1HLA-DRB1基因多态性的表达水平较高,该基因的水平升高会对早发型重度子痫前期孕妇与婴儿预后产生很大的影响,在治疗的期间要对母婴情况进行密切的监测,选择适当的终止妊娠时间,以便提高母婴安全。Objective:To investigate the expression of 1hla-drb1 gene polymorphism in early-onset severe preeclampsia and its effect on pregnancy outcome.Methods:60 patients with early-onset severe preeclampsia admitted to our hospital from February 2018 to February 2019 were selected as the study group,while 60 healthy pregnant women who underwent physical examination in our hospital during the same period were selected as the control group.Human leukocyte antigen gene DRB1(1hla-drb1 gene polymorphism)was detected in the two groups,and the distribution of 1hla-drb1 gene-related SNP genotypes and 1hla-drb1 gene-related SNP genotypes in pregnant women and neonates in the two groups were compared.Results the age,diastolic blood pressure,systolic blood pressure and number of births in the study group were higher than those in the control group(P<0.05).The neonatal birth weight,gestational age,5min and 1minApgar score in the study group were lower than those in the control group(P<0.05),and the incidence of intrauterine distress and asphyxia in the study group was higher than that in the control group(P<0.05).In this study,three SNP sites were found in two gene fragments,namely rs114293611,rs2213586 and rs3135388.Rs2308765 showed no polymorphism in the study.The genotype distribution of rs114293611 in the study group was different from that in the control group(P<0.05),and there was no significant difference in the genotype distribution of rs2213586 and rs3135388 of 1hla-drb1 gene between the two groups(P>0.05).Conclusion:Early hairstyle severe preeclampsia,1 hla-drb1 gene polymorphisms of the expression level is higher,the levels of genes for early hairstyle severe preeclampsia,a large impact on pregnant women and infants prognosis for maternal and infant condition during the period of treatment to close monitoring,select the appropriate time,termination of pregnancy in order to improve maternal and infant safety.
