摘 要:目的探讨建立我国健康人群IOS正常值范围。方法本研究通过对不同关键词组合全面检索中外文数据库(包括万方、CNKI及PubMed数据库)自1997年1月1日到2017年12月31日以来所有相关期刊文献,对符合纳入标准的59篇文献进行分类汇总分析和质量评估、统计分析。结果健康成人(18~82岁,4507例)的呼吸总阻抗、总气道阻力、中心气道阻力、周边气道阻力、弹性阻力以及响应频率分别为(0.380±0.093)kPa·L^-1·s^-1、(0.346±0.099)kPa·L^-1·s^-1、(0.296±0.077)kPa·L^-1·s^-1、(0.059±0.049)kPa·L^-1·s^-1、(-0.108±0.045)kPa·L^-1·s^-1及(12.721±2.987)Hz。健康儿童(1.3~14岁,3336例)的呼吸总阻抗、总气道阻力、中心气道阻力、周边气道阻力、弹性阻力以及响应频率分别为(0.768±0.196)kPa·L^-1·s^-1、(0.729±0.175)kPa·L^-1·s^-1、(0.537±0.151)kPa·L^-1·s^-1、(0.177±0.264)kPa·L^-1·s^-1、(-0.231±0.095)kPa·L^-1·s^-1及(17.045±3.633)Hz。健康儿童IOS正常值均明显高于健康成人(P值均<0.05)。各地域IOS正常值比较,差异均有统计学意义(P值均<0.001)。结论本研究首次总结并归纳分析了我国IOS正常值的范围。我国IOS正常值儿童明显高于成人,不同地域的IOS正常值亦存在差异。目前诊断标准具有相对局限性,建议进行大样本量统一检测标准的多中心研究,建立适合我国人群的正常预计值方程。Objective To explore the establishment of a normal range of IOS for healthy subjects in China.Methods This study searched the relevant studies published in Chinese and foreign journals from January 1,1997 to December 31,2017.Based on the comprehensive search of Chinese and foreign language databases(including Wanfang,CNKI and PubMed databases)with different keyword combination,59 articles that meet the inclusion criteria were classified,summarized,evaluated and statistically analyzed.Results The normal reference value of adults(aged 18~78 yrs,n=4507)was Zrs(0.380±0.093)kPa·L^-1·s^-1,R5(0.346±0.099)kPa·L^-1·s^-1,R20(0.296±0.077)kPa·L^-1·s^-1,R5-R20(0.059±0.049)kPa·L^-1·s^-1,X5(-0.108±0.045)kPa·L^-1·s^-1,Fres(12.721±2.987)Hz.The normal reference value for children(aged 1.3~14 yrs,n=3336)was Zrs(0.768±0.196)kPa·L^-1·s^-1,R5(0.729±0.175)kPa·L^-1·s^-1,R20(0.537±0.151)kPa·L^-1·s^-1,R5-R20(0.177±0.264)kPa·L^-1·s^-1,X5(-0.231±0.095)kPa·L^-1·s^-1,Fres(17.045±3.633)Hz.Children′s normal reference value of IOS were more higher than adults,significantly(all P<0.05).There were significance differences in the normal value of IOS among different regions(all P<0.001).Conclusions This study summarized and analyzed the range of normal values of IOS in China for the first time,which can be used as a reference standard for IOS examination in China.The normal value of IOS in Chinese children is significantly higher than that in adults,and there are differences in the normal value of IOS in different regions.The current diagnostic criteria have relative limitations.It is suggested that a normal predictive value equation suitable for Chinese population should be established by carring on muti center research on unified standard of large samphe size.
