摘 要:目的探讨实时三维超声心动图在左心室容积与收缩功能测量中的可行性及其有效性。方法选取2018年1-12月浙江省中医药大学附属温州市中医院进行健康体检的健康者54例,对其分别进行常规二维双平面Simpson法检查以及实时三维超声心动图检查,分析两种检查方法在左心室射血分数(LVEF)、左心室舒张末期容积(EDV)以及左心室收缩末容积(ESV)的差异,并对图像获取时间及分析时间进行比较。结果实时三维超声心动图LVEF(%)、EDV和ESV检测结果与常规双平面LVEF(%)、EDV和ESV结果差异均无统计学意义(t=0.971、0.154、1.154,均P>0.05)。实时三维超声心动图检测图像获取时间[(90.1±5.4)s]、分析时间[(221.4±22.6)s]均短于常规双平面检测[(129.4±6.7)s、(283.2±23.5)s],两种检测方法比较差异均有统计学意义(t=13.862、14.225,均P<0.05)。结论实时三维超声心动图可测量健康人的左心室容积和收缩功能,是一种无创性检测方法,具有成像速度快、分析结果准确的特点。Objective To investigate the feasibility and effectiveness of real-time three-dimensional echocardiography in the measurement of left ventricular volume and systolic function.Methods From January 2018 to December 2018,54 healthy subjects received physical examination in Wenzhou Hospital of Traditional Chinese Medicine Affliliated to Zhejiang University of Traditional Chinese Medicine were selected.The conventional two-dimensional biplanar Simpson method and real-time three-dimensional echocardiography were performed respectively.The differences of left ventricular ejection fraction(LVEF),left ventricular end-diastolic volume(EDV),end-systolic volume(ESV)between the two kinds of inspection methods were analyzed,and the image acquisition time and analysis time were compared.Results The differences of LVEF(%),EDV,ESV were not statistically significant between real-time three-dimensional echocardiography and conventional biplane echocardiography(t=0.971,0.154,1.154,all P>0.05).The acquisition time and analysis time of real-time three-dimensional echocardiography were shorter than those of conventional two-plane echocardiography[(90.1±5.4)s vs.(129.4±6.7)s,(221.4±22.6)s vs.(283.2±23.5)s],and there were ststistically significant differences between the two methods(t=0.015,0.045,all P<0.05).Conclusion Real-time three-dimensional echocardiography can measure the left ventricular volume and systolic function of patients,and this examination is a non-invasive operation with the characteristics of fast imaging and accurate analysis results,and it is suggested to be popularized and applied in clinical practice.
