摘　　要：目的:探讨红细胞分布宽度与血小板之比(RPR)在评估急性胰腺炎(AP)严重程度的作用及比较其与红细胞分布宽度(RDW)的优劣性。方法:回顾2015年1月到2019年12月南方医科大学珠江医院住院的急性胰腺炎患者145例。根据疾病严重程度分为轻症急性胰腺炎(MAP)、中度重症急性胰腺炎(MSAP)、重症急性胰腺炎(SAP)。收集患者的一般资料、血钙(Ca)、凝血酶原时间(PT)、部分活化凝血酶原时间(APTT)、国际标准化比值(INR)、纤维蛋白原(FIB)、RDW、血小板(PLT)、RPR、平均血小板体积(MPV)、血小板分布宽度(PDW),分析RDW和RPR与AP严重程度的关系。结果:SAP组患者中RPR、RDW、MPV、PDW均明显高于MAP和MSAP组患者,Ca及PLT则低于MAP和MSAP组(P<0.05)。SAP与MSAP组PT、APTT、INR、FIB水平均明显高于MAP组(P<0.05)。RPR和RDW评估SAP的ROC曲线下面积相近,分别为0.809(95%CI 0.721-0.897,P=0.000)和0.808(95%CI 0.729-0.886,P=0.000)。结论:RPR可作为一个有效的评估AP严重程度的指标,其与RDW的作用相当。Objective:To investigate the role of red cell distribution width(RDW)to platelet(PLT)ratio(RPR)in severity assessment of acute pancreatitis(AP)and to compare the advantages and disadvantages between RPR and RDW.Methods:A total of 145 case of acute pancreatitis treated in Zhujiang Hospital of Southern Medical University between January 2015 and December 2019 were reviewed.According to the disease severity,these cases were categorized as mild acute pancreatitis(MAP),moderate-to-severe acute pancreatitis(MSAP)and severe acute pancreatitis(SAP).The patients were investigated on general data,serum calcium(Ca),prothrombin time(PT),partially activated prothrombin time(APTT),international normalized ratio(INR),fibrinogen(FIB),RDW,PLT,RPR,Mean platelet volume(MPV),and platelet distribution width(PDW).The correlation of RDW and RPR with AP severity was analyzed.Results:The patients in SAP group showed significantly higher RPR,RDW,MPV and PDW,as well as lower serum Ca and PLT count,compared with those in MAP and MSAP groups(P<0.05).The values of PT,APTT,INR and FIB in SAP and MSAP groups were significantly higher than those in MAP group(P<0.05).The area under ROC curve of RPR and RDW was similar,being 0.809(95%CI 0.721-0.897,P=0.000)and 0.808(95%CI 0.729-0.886,P=0.000),respectively.Conclusion:RPR can be used as an effective index to evaluate the severity of AP,with equivalent performance of RDW.
