摘  要:目的观察补肺颗粒对慢性阻塞性肺疾病(COPD)稳定期大鼠肺功能、右心及肺动脉超声以及肺组织病理的改善情况,评价补肺颗粒对COPD稳定期模型大鼠肺血管病变的疗效。方法将40只雄性SD大鼠随机分为空白组、模型组、罗氟司特组(西药组)、补肺颗粒组(中药组),每组10只。除空白组外,其余各组采用熏香烟结合气道内滴注内毒素法制造COPD稳定期大鼠模型,造模80 d后,各组大鼠静养20 d进入稳定期,至此造模完毕。之后空白组和模型组给予生理盐水灌服,罗氟司特组与补肺颗粒组分别给予罗氟司特和补肺颗粒治疗,持续30 d。测定各组大鼠肺功能[用力肺活量(FVC)、最大呼气量(FEV 0.3)、FEV 0.3/FVC],并分别于实验第30、60、80、130 d以小动物超声测定各组大鼠PR Peak(肺动脉瓣反流)、肺动脉血流速度(PV Peak)、平均肺动脉压(mPAP)及制作肺组织病理切片。结果造模期间,空白组大鼠发育良好,模型组毛发干枯少光泽,神情倦怠,体重明显减轻;干预期间,空白组及模型组较干预前无明显改变,西药组和中药组改善明显,活动增加,反应变快,饮食增多,体质量回升。造模后,与空白组比较,其余3组在肺功能各项指标方面均有降低(P<0.05),超声各项指标均有不同程度的升高(P<0.05);与空白组比较,各造模大鼠COPD模型大鼠支气管结构出现一定程度损伤,黏膜下出现大量炎细胞浸润,肺泡结构紊乱扩张融合形成肺大泡,肺血管增生,管壁增厚,官腔变形,微血管数量增多;经过30 d给药干预后,与模型组比较,西药组及中药组肺功能各项指标均明显升高(P<0.01),超声各项指标均明显下降(P<0.01);与模型组比较,西药组及中药组大鼠肺组织中各损伤均有明显改善。结论补肺颗粒可以明显改善COPD稳定期大鼠肺功能,提高其右心及肺动脉功能,并一定程度修复肺组织损伤及抑制肺血管增生重塑,保护肺血管,对预防COPD肺Objective To observe the effect of Bufei Granule on pulmonary function,right heart and pulmonary artery ultrasonography and pulmonary pathology in rats with stable chronic obstructive pulmonary disease(COPD),and to evaluate the effect of Bufei Granule on pulmonary vascular disease in COPD stable rats.Methods Forty male SD rats were randomly divided into blank group,model group,Rofluast group and Bufei Granule group with 10 rats in each group.The stable COPD rat model was made by smoking cigarette combined with endotoxin intratracheal drip.After 80 days,the rats in each group were rested for 20 days,and then the model was completed.After that,the blank group and the model group were given saline infusion,while the Rofloast group and Bufei Granule group were given Rofloast and Bufei Granule respectively for 30 days.Pulmonary function(FVC,FEV 0.3,FEV 0.3/FVC)of rats in each group was measured.PR Peak(pulmonary valve regurgitation),PV Peak(pulmonary artery blood flow velocity),mPAP(mean pulmonary artery pressure)and pathological section of pulmonary tissue were measured by small animal ultrasound at 30,60,80 and 130 days respectively.Results Compared with the blank group,the other three groups had significantly lower lung function indexes(P<0.01)and higher ultrasound indexes(P<0.01).Compared with the blank group,the bronchial structure of COPD model rats in each model group was damaged to a certain extent,a large number of inflammatory cells infiltrated under the mucosa,alveolar structure disorder,dilatation and fusion formed alveoli,and pulmonary vessels increased.Compared with the model group,the lung function indexes of Rofluast group and Bufei Granule group were significantly increased(P<0.01),and the ultrasound indexes were significantly decreased(P<0.01).Compared with the model group,the lung injury of Rofluast group and Bufei Granule group were significantly improved.Conclusions Bufei Granule can significantly improve the pulmonary function of COPD stable rats,improve the function of right heart and pulmonary
