股骨近端防旋髓内钉术配合中医辨证治疗对股骨粗隆间骨折患者髋关节功能水平、生活质量的影响

摘　　要：目的探讨股骨近端防旋髓内钉术(PFNA内固定术)配合中医辨证治疗对股骨粗隆间骨折患者髋关节功能水平、生活质量的影响。方法选取2015年1月—2018年1月期间收治的102例股骨粗隆间骨折患者,按照治疗方法不同分为对照组和研究组,每组51例。对照组给予PFNA内固定术进行治疗,研究组实施PFNA内固定术配合中医辨证治疗,观察并比较两组患者的临床疗效、术后负重时间、骨痂生成时间、股骨粗隆间骨折愈合时间、髋关节功能水平、生活质量以及不良反应等指标。结果研究组治疗后总有效率高于对照组(P<0.05),差异具有统计学意义。研究组治疗后骨痂生成时间、住院时间、术后负重时间以及骨折愈合时间均优于对照组(P<0.05),髋关节功能水平、生活质量均高于对照组(P<0.05),差异具有统计学意义。研究组不良反应总发生率低于对照组(P<0.05),差异具有统计学意义。结论PFNA内固定术配合中医辨证治疗股骨粗隆间骨折不仅能提高患者的临床疗效,减少骨痂生成时间、住院时间、术后负重时间以及骨折愈合时间,还可以改善患者的髋关节功能水平、生活质量及降低其不良反应发生率,值得临床上推广使用。Objective To explore the effect of Proximal Femoral Nail Anti-rotation(PFNA)internal fixation combined with TCM syndrome differentiation on hip function and quality of life in patients with femoral intertrochanteric fracture.Methods 102 patients with intertrochanteric fracture of femur admitted to our hospital from January 2015 to January 2018 were divided into control group and research group according to different treatment methods,with 51 cases in each group.The control group was treated with PFNA internal fixation,while the research group was treated with PFNA internal fixation combined with TCM syndrome differentiation.And to observe and compare the clinical efficacy,hip function,quality of life and adverse reactions of the two groups.Results The total effective rate of the study group(92.2%)was higher than that of the control group(80.4%)with statistical significance(P<0.05).The callus formation time,hospitalization time,weight-bearing time after operation and fracture healing time in the study group were better than those in the control group,with statistical significance(P<0.05).After treatment,the functional level and quality of life of the hip joint in the study group were higher than those in the control group,with statistical significance(P<0.05).The adverse reactions in the study group(6.0%)were lower than those in the control group(16.0%)with statistical significance(P<0.05).Conclusions For patients with intertrochanteric fracture of femur,PFNA internal fixation combined with TCM syndrome differentiation treatment can improve the clinical efficacy,reduce callus formation time,hospital stay,weight-bearing time after operation and fracture healing time,improve the functional level of hip joint,quality of life and reduce the incidence of adverse reactions.It is worthy of clinical application.
