新生儿海洛因戒断综合征的临床分析

摘　　要：目的:对新生儿海洛因戒断综合征进行临床分析。方法:2006年9月-2013年6月收治新生儿海洛因戒断综合征患儿20例,在抢救危重症的同时采取综合治疗。观察患儿临床症状、戒断综合征相关性因素及治疗后转归情况。结果:20例患儿除原发病外,出现呕吐、腹胀16例(80%),流鼻涕、打哈欠、流眼泪20例(100%),烦躁不安14例(70%),强吸吮、啃手指16例(80%)。母体吸毒时间与新生儿临床症状出现时间、Finnegan量表评分间无相关性关系。母体末次吸毒时间与新生儿临床症状出现时间、Finnegan量表评分对比存在相关性关系,末次吸毒时间与新生儿娩出时间越接近,新生儿临床症状越晚出现,Finnegan量表评分越高。20例患儿均痊愈出院,随访5例,其中1例吵闹不安到5个月后好转,其余4例患儿随访至今未见智力及行为异常。结论:新生儿海洛因戒断综合征患儿临床症状表现复杂,母亲末次吸毒时间距分娩时间越长,患儿出现临床症状越早,且评分越低,需要引起高度重视。新生儿科与妇产科医生可配合对患儿进行危重症抢救及综合治疗,转归效果理想。Objective:To analyze neonatal heroin withdrawal syndrome.Methods:From September 2006 to June 2013,20 cases of neonatal heroin withdrawal syndrome were enrolled.To take comprehensive treatment while rescuing critical illness.The clinical symptoms,related factors of withdrawal syndrome and outcome after treatment were observed.Results:In addition to the primary disease of 20 cases,16 cases(80%)had vomiting and abdominal distension,20 cases(100%)had runny nose,yawning and tears.14 cases(70%)were restlessness;16 cases(80%)had strong sucking and gnawing fingers.There was no correlation between the time of maternal drug abuse and the appearing time of clinical symptoms of neonates and the Finnegan score scale.A correlation existed between the last time of maternal drug abuse and the appearing time of clinical symptoms of neonates and the Finnegan scale score,as the last time of drug abuse was closer to the time of delivery,the later clinical symptoms appeared and the higher the Finnegan scale score.All 20 patients were cured and discharged from hospital.Five of them were followed up.One of them improved noisy after 5 months,the other 4 cases did not have abnormal intelligence and behavior so far.Conclusion:The clinical symptoms of neonatal heroin withdrawal syndrome are complicated,the longer the time of mother's last drug abuse to the time of delivery,the earlier the children's clinical symptoms appear,the lower the score is,it needs to be highly valued.Doctors in neonatal department and gynaecology and obstetrics department can cooperate with the children for critical care and comprehensive treatment,the outcome effect is ideal.
