摘 要:目的对比分析微型空心螺钉及交叉克氏针在末节指骨骨折治疗中的临床效果。方法回顾性分析2016年6月至2019年3月手术治疗并获得完整随访的146例末节指骨骨折患者的资料,根据手术方式的不同分为交叉克氏针组(n=79)和微型空心螺钉组(n=67),分析2组患者的手术时间、术中透视次数、骨折愈合情况、术后患指功能恢复情况及并发症发生率等指标。结果微型空心螺钉组治疗成人末节指骨骨折手术时间、骨折愈合率、患指功能优良率均优于交叉克氏针组(P<0.05);术后并发症发生率及术中透视次数显著低于交叉克氏针组(P<0.05)。结论微型空心螺钉治疗成人末节指骨骨折,精准复位及微创固定的特点,其手术时间短、创伤小,在保证骨折复位质量及愈合率的同时,可显著提高患指功能。Objective To investigate the therapeutic effects of miniature hollow screws and crossed Kirschner wires in treatment of end-phalangeal fractures.Methods The clinical data about 146 patients with distal phalangeal fractures who were treated by surgery in our hospital from June 2016 to March 2019 were retrospectively analyzed.According to different surgical modes,these patients were divided into crossed Kirschner wire group(n=79)and miniature hollow screws group(n=67).The operative time,intraoperative fluoroscopy frequency,fracture healing status,postoperative finger functional recovery and the incidence rate of complication were observed and compared between the two groups.Results The operation time,fracture healing rate,and good/excellent rate of finger function in miniature hollow screws group were superior to those of crossed Kirschner wire group(P<0.05).However the incidence rates of intraoperative fluoroscopy frequency and incidence rate of postoperative complications in miniature hollow screws group were significantly lower thant hose in crossed Kirschner wire group(P<0.05).Conclusion The miniature hollow screws in treatment of distal phalangeal fracture in adults has the advantages of precise reduction and minimally invasive fixation,with shorter operation time and less invasiveness,which can significantly improve the function of affected fingers and ensure the quality of fracture reduction and healing.
