痰标本真菌检验阳性结果的研究及分析

摘　　要：目的研究与分析痰标本真菌检验阳性结果对诊断疾病的价值和意义。方法选取我院各个科室住院患者的500例痰液标本为对象进行研究,通过回顾性分析其全部的临床资料,对所有标本进行革兰氏染色,并采用显微镜进行检查。观察指标为侵袭性肺部感染的确诊率和真菌阳性率。结果500例标本中,丝状霉菌总阳性率为19.6%(98/500),其中包括烟曲霉36例、黄曲霉34例、黑曲霉20例、毛霉菌8例;酵母样真菌的阳性率为36.6%(183/500),其中包括白色念珠菌68例、热带念珠菌50例、光滑念珠菌23例、克柔念珠菌30例、其他酵母样真菌12例;丝状霉菌的确诊率为56.12%(55/98)、酵母样真菌的确诊率为11.48%%(21/183)。结论在临床诊断和判断疾病方面,痰标本真菌检验阳性结果具有一定临床意义,而与酵母样真菌的阳性结果相比,丝状霉菌的阳性结果更具临床价值。Objective To study and analyze the value and significance of positive results of fungi test in sputum specimens for the diagnosis of diseases.Methods 500 sputum specimens from inpatients in various departments of our hospital were selected for study.All the clinical data were retrospectively analyzed.All specimens were stained by Gram's staining and examined by microscopy.The diagnostic rate of invasive pulmonary infection and the positive rate of fungi were observed.Results The total positive rate of filamentous fungi was 19.6%(98/500),including Aspergillus fumigatus 36 cases,Aspergillus flavus 34 cases,Aspergillus Niger 20 cases and Mucor 8 cases.The positive rate of yeast-like fungi was 36.6%(183/500),including 68 cases of Candida albicans,50 cases of Candida tropicalis,23 cases of Candida glabrata,30 cases of Candida krussiana and 12 cases of other yeast-like fungi.The diagnostic rate was 56.12%(55/98)and that of yeast-like fungi was 11.48%(21/183).Conclusion In clinical diagnosis and judgment of diseases,the positive results of fungal test in sputum specimens have certain clinical significance.Compared with the positive results of yeast-like fungi,the positive results of filamentous fungi have more clinical value.
