摘　　要：目的观察麝香乌龙丸联合双醋瑞因胶囊治疗风寒湿痹型膝骨关节炎(KOA)临床疗效及对患者血清白细胞介素6(IL-6)、基质金属蛋白酶1(MMP-1)、基质金属蛋白酶3(MMP-3)、金属蛋白酶组织抑制剂1(TIMP-1)水平的影响。方法将80例风寒湿痹型KOA患者按照随机数字表法分为2组。对照组40例予双醋瑞因胶囊治疗;治疗组40例在对照组治疗基础上加用麝香乌龙丸治疗。2组均治疗12周。观察2组治疗前后症状量化评分、膝关节压痛视觉模拟评分(VAS)、20 m步行痛VAS变化,以及治疗前后血清IL-6、MMP-1、MMP-3、TIMP-1水平变化。结果治疗后2组症状量化评分、膝关节压痛VAS、20 m步行痛VAS均降低(P<0.05),且治疗组均低于对照组(P<0.05)。治疗后2组血清IL-6、MMP-1、MMP-3水平均下降(P<0.05),且治疗组均低于对照组(P<0.05)。治疗后2组血清TIMP-1水平均升高(P<0.05),且治疗组高于对照组(P<0.05)。结论麝香乌龙丸联合双醋瑞因胶囊能减轻风寒湿痹型KOA患者关节症状,改善关节功能,可能与降低IL-6、MMP-1、MMP-3水平及提高TIMP-1水平相关。Objective To observe the effect of Shexiang Wulong pills combined with diacerein capsules on knee osteoarthritis(KOA)of wind-cold-damp retention type and its effect on serum interleukin 6(IL-6),matrix metalloproteinase 1(MMP-1),matrix metalloproteinase 3(MMP-3)and tissue inhibitor of metalloproteinase 1(TIMP-1).Methods 80 with KOA patients were divided into two groups according to the random number table method.40 cases in control group were treated by diacerein capsules.40 cases in treatment group were treated by Shexiang Wulong pills on the basis of the control group.Both groups were treated for 12 weeks.The changes of symptoms quantification score,visual analogue score(VAS)of knee joint tenderness,VAS of 20 m walking pain,and the changes of serum IL-6,MMP-1,MMP-3 and TIMP-1 were observed between two groups before and after treatment.Results After treatment,the quantitative score of symptoms,VAS of knee joint tenderness and VAS of 20 m walking pain in the two groups were decreased(P<0.05),and the treatment group was lower than the control group(P<0.05).After treatment,the levels of IL-6,MMP-1 and MMP-3 in the two groups were decreased(P<0.05),and the treatment group was lower than the control group(P<0.05).After treatment,the level of TIMP-1 in both groups was increased(P<0.05),and the treatment group was higher than the control group(P<0.05).Conclusion Shexiang Wulong pills combined with diacerein capsules can reduce the symptoms and improve the joint function of KOA patients with wind-cold-damp retention type,which may be related to the decrease of IL-6,MMP-1,MMP-3 and the increase of TIMP-1.
