经阴道内镜保胆取石术的技术初探

摘　　要：目的:探讨经阴道内镜保胆取石术的手术方法及有效性、安全性。方法:收集2018年11月至2019年4月经阴道行内镜保胆取石术的7例胆囊结石患者的临床资料,患者27~51岁,平均(39.9±8.5)岁,分析手术时间、术中出血量、术后卧床时间、疼痛评分、住院时间等。结果:7例患者均成功施行经阴道内镜保胆取石术,手术时间45~70 min,平均(56.0±9.0)min;术中出血量10~25 mL,平均(15.7±6.1)mL;术后下床活动时间6~12 h,平均(8.0±2.3)h,术后住院3~5 d,平均(3.7±0.7)d。术后1例患者出现一过性发热,经对症治疗后好转,未发生其他并发症。结论:经阴道内镜保胆取石术是安全、有效的,在自然通道建立、闭合与胆囊显露等方面均具有优势。Objective:To investigate the safety and efficiency of the transvaginal endoscopic gallbladder-preserving cholelithotomy.Methods:A retrospective study was performed on the clinical data of 7 patients with cholecystolithiasis who underwent transvaginal endoscopic gallbladder-preserving cholelithotomy from Nov.2018 to Apr.2019.The age of patients was 27-51 years old with the average of(39.9±8.5).The surgery time,intraoperative blood loss,postoperative bedrest time,pain scores,hospital stay and so on were analyzed.Results:Seven patients were successfully implemented the transvaginal endoscopic gallbladder-preserving cholelithotomy.The average time of surgery was(56.0±9.0)min(range,45-70 min).The average intraoperative blood loss was(15.7±6.1)mL(range,10-25 mL),the postoperative ambulation time was 6-12 h with the average of(8.0±2.3)h,and the average postoperative hospital stay was(3.7±0.7)d(range,3-5 d).1 patient suffered from transient fever and recovered after symptomatic treatment.No other complications were found.Conclusions:The transvaginal endoscopic gallbladder-preserving cholelithotomy is safe and effective,it has advantages in establishment and closure of natural orifice and gallbladder exposure.
