摘　　要：目的探讨脑囊虫病患者脑脊液和血清IgE、IgG水平及其临床意义。方法选取2013年6月至2018年6月丰都县人民医院及陆军军医大学第二附属医院收治的脑囊虫病患者46例作为脑囊虫病组,同期收治的脑血管疾病患者40例作为对照组,比较两组脑脊液和血清IgE、IgG水平及补体C3、C4水平,分析脑脊液IgE、IgG与血清IgE、IgG水平间的关系,采用受试者工作特征曲线(ROC)分析脑脊液及血清IgE、IgG诊断脑囊虫病的价值。结果脑囊虫病组脑脊液及血清IgE、IgG水平均高于对照组,差异有统计学意义(t脑脊液=5.576,6.339,P<0.001;t血清=2.360,3.112,P=0.021,0.003);脑囊虫病组脑脊液及血清C3、C4水平均低于对照组,差异有统计学意义(t脑脊液=3.679,4.599,P<0.001;t血清=2.187,2.621,P=0.032,0.010);脑脊液IgE与血清IgE呈正相关(r=0.792,P<0.05);脑脊液IgG与血清IgG呈正相关(r=0.834,P<0.05);脑脊液IgG诊断脑囊虫病效能最高,临界值为0.208 g/L,敏感度为89.10%,特异度为86.50%。结论脑囊虫病患者脑脊液和血清IgE、IgG水平均明显升高,补体C3、C4水平明显降低,且脑脊液IgG对诊断脑囊虫病有辅助价值。Objective To investigate the levels of cerebrospinal fluid and serum IgE and IgG in patients with cerebral cysticercosis and their clinical significance.Methods 46 patients with cerebral cysticercosis admitted to People's Hospital of Fengdu County and the Second Affiliated Hospital of the Army Medical University from June 2013 to June 2018 were selected as the cerebral cysticercosis group.And 40 patients with cerebrovascular disease admitted during the same period were taken as the control group.The levels of cerebrospinal fluid and serum IgE and IgG,complements C3 and C4 were compared between the two groups.The relationship between IgE and IgG levels in cerebrospinal fluid and in serum was analyzed.The value of cerebrospinal fluid and serum IgE and IgG in the diagnosis of cerebral cysticercosis was analyzed by the receiver operating characteristic(ROC)curve.Results The levels of IgE and IgG in cerebrospinal fluid and serum of the cerebral cysticercosis group were higher than those in the control group(tcerebrospinal fluid=5.576,6.339,P<0.001;tserum=2.360,3.112,P=0.021,0.003).The levels of cerebrospinal fluid and serum C3 and C4 in the cerebral cysticercosis group were lower than those in the control group,and the difference was statistically significant(tcerebrospinal fluid=3.679,4.599,P<0.001;tserum=2.187,2.621,P=0.032,0.010).Cerebrospinal fluid IgE was positively correlated with serum IgE(r=0.792,P<0.05).And cerebrospinal fluid IgG was positively correlated with serum IgG(r=0.834,P<0.05).Cerebrospinal fluid IgG was the most effective in the diagnosis of cerebral cysticercosis,with a critical value of 0.208 g/L,a sensitivity of 89.10%,and an specificity of 86.50%.Conclusion The levels of cerebrospinal fluid,serum IgE and IgG in patients with cerebral cysticercosis were significantly increased,while the levels of complement C3 and C4 were significantly decreased.Cerebrospinal fluid IgG can be used in the diagnosis of cerebral cysticercosis.
