摘 要:目的探讨丙烯腈(ACN)接触工人的血管内皮功能。方法选取长期接触ACN作业工人57人[男性31人,女性26人;平均年龄(46.3±7.7)岁]作为接触组,同时选取不接触任何毒物的普通工人53名[男性29人,女性24人;平均年龄(47.3±8.8)岁]作为对照组,采用GE Vivid超声诊断仪进行常规超声心动图及肱动脉血管内皮功能检查,并计算血流介导的血管舒张功能(flow-mediated dilation,FMD)和非内皮依赖的血管舒张功能(nitroglycerin-mediated dilation,NMD)。结果两组间常规超声心动图指标无明显差异,接触组FMD、NMD明显低于对照组,并与ACN接触时间呈负相关(r=-0.519,-0.583,P均<0.05)。结论ACN对血管内皮功能存在不同程度的功能损伤,而且随着ACN接触时间延长而降低。肱动脉内皮功能检查可有效评价ACN接触工人的血管内皮功能。Objective To investigate the endothelial function in occupational acrylonitrile exposure workers.Methods 57workers of occupational exposure to acrylonitrile(31males,26females,mean age 46.3±7.7years)were choosed as exposure group and 53ordinary workers(29males,24females,mean age 47.3±8.8years)without any occupational exposure history of toxicant were choosed as control group.All participants underwent conventional echocardiography.The brachial artery flow-mediated dilation(FMD)and nitroglycerin-mediated dilation(NMD)were measured and calculated by ultrasound to assess endothelial function.All data were analyzed with SPSS 19.0statistical software,and the P<0.05was considered to indicate a statistically significant result.Results There was no significant difference in traditional echocardiographic parameters between the two groups.In the exposure group,the FMD and NMD were significantly lower than those in the control group.Besides,the FMD and NMD were negatively corrlated with acrylonitrile contact time(r=-0.519,-0.583,respectively,all P<0.05).Conclusions There was different degree impairment of endothelial function in occupational acrylonitrile exposure workers,which decreased as the exposure duration.The brachial artery FMD and NMD can effectively detect the endothelial function in workers exposed to acrylonitrile.
