摘 要:目的探讨长期职业性噪声暴露与外周血细胞端粒相对长度(relative telomere length,RTL)的相关性。方法选择某石化企业工龄≥4a且个体噪声暴露40h等效声级(LEX,周)≥80dB(A)的52名男性工人作为研究对象的暴露组,(LEX,周)<80dB(A)的50名男性工人作为对照组。通过问卷调查和健康检查获得研究对象的一般情况、职业史和血常规检测结果,应用荧光定量PCR法检测研究对象的RTL,采用广义线形回归法分析职业暴露噪声强度与RTL的相关性。结果噪声暴露组和对照组工人除年龄和工龄外,在吸烟、饮酒人数、体质量指数(body mass index,BMI)、血常规以及苯系物累积暴露水平等方面差异均无统计学意义(P>0.05)。两组工人RTL的检测值呈偏态分布,以中位数(四分位数)表示,分别为暴露组1.31(1.16,1.55)和对照组1.34(1.22,1.63),经广义线性多元回归法校正影响因素后显示两组差异有统计学意义(P<0.05)。按(LEX,周)是否大于85dB(A)将暴露组进一步分组后发现,(LEX,周)≥85dB(A)组工人RTL明显短于对照组(P<0.05),且RTL随职业噪声暴露强度增加而减少(P<0.05)。结论长期职业噪声暴露可能影响外周血细胞RTL。Objective To investigate the association between occupational noise exposure and relative telomere length(RTL)of peripheral blood cells.Methods 52male workers whose exposure duration to noise was longer than 4years and the individual noise exposure intensity(LEX,w)≥80dB(A)were selected in the noise group,50male workers whose exposure duration to noise less than 80dB(A)(LEX,w)was longer than 4 years were selected in the control group.Via questionnaires and routine blood test,the general,occupational and routine blood information of the two groups was obtained.Real-time fluorescence quantitative polymerase chain reaction(PCR)was applied to detect RTL of peripheral blood cells.The generalized linear regression method was used for analyzing the correlation between occupational noise intensity and RTL.Results In this study,except age and exposure duration,no statistical difference was observed in smoking,drinking,body mass index(BMI),blood routine or benzene content cumulative exposure levels between the two groups.The values of RTL were skewed distribution.The median(quartile)of RTL in the noise and control group were 1.31(1.16,1.55)and 1.16(1.22,1.22).Adjust the confounding factors by the generalized linear multivariate regression,the difference of RTL between the two groups was statistically significant(P<0.05).Further analysis indicated that RTL of the(LEX,w)>85dB(A)group was shorter than the control group(P<0.05),and the value of RTL decreased with the increase of occupational noise exposure intensity(P<0.05).Conclusions Long-term occupational noise exposure might affects the telomere length of peripheral blood cells.
