艾滋病多学科协作诊治模式的实施与成效

摘　　要：高效抗反转录病毒治疗(highly active antiretroviral therapy, HAART)改变了艾滋病的进程及其患者的预后,同时也改变了艾滋病患者的疾病谱,促使人们相应调整对艾滋病患者的关怀和诊治模式。在HAART出现之前和HAART时代,人们对艾滋病患者的关注重点是机会性感染、艾滋病相关肿瘤的诊治和HAART本身;而在后HAART时代,非艾滋病定义性疾病成为对艾滋病患者关怀的主要内容。因此,艾滋病诊治模式亦已从以感染科医生为主的诊治模式逐渐转变为感染科医生参与的多学科协作诊治模式。本文报告上海市公共卫生临床中心艾滋病多学科协作诊治模式的建立背景、运行方式和实施成效,总结多学科协作诊治模式在艾滋病综合诊治中的作用和价值,为我国推广实施艾滋病多学科协作诊治模式提供参考。Highly active antiretroviral therapy(HAART)alters the course of AIDS and prognosis of patients,and meanwhile,it changes the disease spectrum of AIDS patients,in which people make corresponding adjustments in the mode of care and treatment of AIDS.Prior to the application of HAART and in the era of HAART,the priority was given to opportunistic infections,the treatment of AIDS-related tumors and HAART.In the post-HAART era,non-AIDS-defining diseases have dominated AIDS care,and accordingly the traditional diagnosis and treatment mode has gradually been substituted by the mode featuring multi-disciplinary treatment(MDT)composed of infectious disease physicians.This paper reports the background,operation and implementation effect of MDT mode for AIDS diagnosis and treatment in Shanghai Public Health Clinical Center,summarizes the role and value of MDT mode in AIDS diagnosis and treatment,and puts forward suggestions for the further implementation of MDT mode for AIDS in China.
