输尿管镜气压弹道碎石术治疗54例输尿管上段结石患者的疗效观察

摘　　要：目的 :探讨输尿管镜气压弹道碎石术(ureteroscopic pneumatic lithotripsy, URSL)对嵌顿性输尿管上段结石的治疗效果及对血清炎症因子水平的影响。方法 :回顾性分析114例嵌顿性输尿管上段结石患者,根据患者手术方式分为URSL组(n=54)和经皮肾穿刺取石术组(n=60)。观察两组手术一般情况,比较手术前后血清炎症因子水平、排尿功能和生活质量的差异。结果:URSL组手术时间、术中出血量、住院时间及结石残留率均低于经皮肾穿刺取石组(P<0.05);URSL组术后1周CRP、TNF-a、NF-κB及IL-1水平均低于经皮肾穿刺取石术组(P <0.05);URSL组术后排尿功能和生活质量均优于经皮肾穿刺取石术组(P <0.05)。结论:URSL对嵌顿性输尿管上段结石有较好的治疗效果,可降低患者术后血清炎症因子水平水平,提高生活质量。Objective:To investigate the effect of ureteroscopic pneumatic lithotripsy(URSL)on incarcerated upper ureteral calculi and serum inflammatory factors.Methods:One hundred and fourteen patients with incarcerated upper ureteric calculi were retrospectively analyzed and divided into a URSL group(n=54)and a percutaneous nephrolithotomy group(n=60)according to their surgical methods.The general condition,the difference of serum inflammatory factors,the urination function and the quality of life before and after the operation were observed and compared between the two groups.Results:The operation time,the intraoperative blood loss,the hospitalize time and the stone residual rate and the levels of CRP,TNF-a,NF-κB and IL-1 were lower and the postoperative urinary function and the quality of life were better in the URSL group than the percutaneous nephrolithotomy group(P<0.05).Conclusion:URSL has a good therapeutic effect on incarcerated upper ureteral calculi and can reduce the level of serum inflammatory factors and improve the quality of life.
