药物涂层球囊处理冠状动脉左前降支开口原位病变的临床疗效

摘　　要：目的观察药物涂层球囊处理左前降支开口原位病变的疗效和安全性。方法2016年1月至2018年12月期间因左前降支开口原位病变接受药物涂层球囊治疗后患者22例,分析患者的临床资料、术前与术后冠状动脉造影,术后随访结果。结果22例患者药物涂层球囊治疗即刻手术成功率为100%,最小管腔直径从术前(0.77±0.42)mm增加至术后的(2.16±0.49)mm,狭窄程度从术前的(77.59±9.97)%减少到术后(22.05±7.34)%,差异均有统计学意义(均P<0.01)。术后2年内16例患者接受冠状动脉造影复查,其中2例患者出现靶病变再狭窄,进行靶病变血运重建,其余病例均未发生主要心血管不良事件。16例冠状动脉造影复查显示靶病变最小管腔直径[(2.38±0.69)mm]及狭窄程度[(20.56±16.65)%],与术后即刻相应测值比较均有改善,但差异无统计学意义(P>0.05)。结论药物涂层球囊治疗左前降支开口原位病变安全有效。Objective To observe clinical efficacy and safety of drug-coated balloon(DCB)in treating de-novo ostial lesion of left anterior descending coronary artery(LAD).Methods Clinical data,coronary angiography(CAG)before and after coronary intervention,and prognosis during follow-up were reviewed retrospectively in 22 patients underwent DCB treatment in de-novo ostial lesion of LAD from January 2016 to December 2018.Results The immediate success rate of DCB treatment was 100%in all 22 patients.The minimum lumen diameter increased significantly from(0.77±0.42)mm before to(2.16±0.49)mm after the procedure(P<0.01).The stenosis degree decreased significantly from(77.59±9.97)% before to(22.05±7.34)% after the procedure(P<0.01).During follow-up 2 years,16 patients underwent repeat CAG.Two of them had target lesion restenosis and underwent target lesion revascularization,while the remaining cases had no major adverse cardiovascular events.Repeat CAG in 16 cases showed the minimum lumen diameter[(2.38±0.69)mm]and stenosis degree[(20.56±16.65)%]of the target lesion were further improved but without significantly different to those immediately after DCB treatment(P>0.05).Conclusion DCB is efficacy and safety in the treatment of de-novo ostial lesion of LAD.
