正畸-正颌联合治疗骨性Ⅲ类错[牙合]的应用分析

摘　　要：目的分析正畸-正颌联合治疗骨性Ⅲ类错[牙合]畸形的疗效,为后续临床应用提供参考。方法便利选取2014年1月—2018年12月在武汉大学口腔医院及厦门医学院附属口腔医院接受正畸-正颌联合治疗的骨性Ⅲ类错[牙合]畸形患者为研究对象,治疗前、治疗后3个月及治疗后1年后使用X线头颅定位片分析患者的颌面部16项角度指标。结果治疗后3个月与之前比较,N-S-Ar、N-SBa、N-SGo、NBa-PtGn以及Y轴差异无统计学意义(P>0.05),差异有统计学意义(P<0.05);治疗后3个月与治疗后1年比较SNPo分别为(83.63±2.02)°、(85.60±2.45)°;U1-SN分别为(113.23±5.36)°、(117.56±5.23)°,差异有统计学意义(P<0.05),其余差异无统计学意义(P>0.05)。结论正畸-正颌治疗骨性Ⅲ类错[牙合]畸,虽然在术后一定时间内会存在组织测量角度的变化,但未对手术效果造成影响,该种治疗方式安全可靠。Objective To analyze the curative effect of orthodontic-orthognathic combined treatment of skeletal typeⅢmalocclusion,and to provide a reference for subsequent clinical application.Methods From January 2014 to December 2018,patients with typeⅢmalocclusion who underwent combined orthodontics-orthognathic treatment at the Stomatological Hospital of Wuhan University and the Affiliated Stomatological Hospital of Xiamen Medical College were conveniently selected as the research subjects,before and after treatment.Three months and one year after treatment,X-ray skull positioning films were used to analyze 16 angle indexes of the maxillofacial region of the patients.Results Compared with the previous 3 months after treatment,the differences between NS-Ar,N-SBa,N-SGo,NBa-PtGn and Y axis were not statistically significant,and the remaining differences were statistically significant(P<0.05);1 year after treatment and 3 months after treatment comparison of SNPo was(83.63±2.02)°and(85.60±2.45)°;U1-SN was(113.23±5.36)°and(117.56±5.23)°.The changes were statistically significant different(P<0.05),and the rest were not statistically significant(P>0.05).Conclusion Orthodontic-orthognathic treatment of skeletal typeⅢmalocclusion,although there will be changes in the angle of tissue measurement within a certain period of time after surgery,but it does not affect the surgical effect,this treatment method is safe and reliable.
