摘 要:目的:探讨有创序贯无创机械通气治疗COPD呼吸衰竭的临床疗效。方法:选取2017年7月~2019年6月在本院治疗的COPD呼吸衰竭患者共80例为研究对象,分为观察组和对照组各40例。所有患者均给予解痉、平喘、抗感染、扩张支气管等对症治疗,并积极治疗原发病。同时,对照组患者接受气管插管机械通气治疗,观察组患者接受有创序贯无创机械通气治疗。对比两组患者机械通气时间、ICU及总住院时间,以及抢救成功率、再次气管插管率、VAP发生率等。结果:观察组患者相关治疗时间(机械通气、ICU住院、住院)均明显短于对照组(P<0.05);观察组患者的抢救成功率明显高于对照组,VAP发生率低于对照组(P<0.05);两组患者的再插管率无明显差异(P>0.05)。结论:有创序贯无创机械通气治疗可更加快速的控制患者病情,提高抢救效果,且能够减少VAP发生,更具应用价值。Objective:To explore the clinical effect of invasive sequential noninvasive mechanical ventilation in the treatment of COPD respiratory failure.Methods:Select from July 2017 to June 2019,a total of 80 COPD patients with respiratory failure treated in our hospital were selected as the study subjects and divided into observation group and control group with 40 cases each.All patients were given symptomatic treatment such as spasmolysis,asthma,anti-infection,and bronchiectasis,and actively treated the primary disease.At the same time,patients in the control group received tracheal intubation mechanical ventilation,and patients in the observation group received invasive sequential noninvasive mechanical ventilation.The mechanical ventilation time,ICU and total hospitalization time,the success rate of resuscitation,the rate of reintubation,and the incidence of VAP were compared between the two groups.Results:The related treatment time(mechanical ventilation,ICU hospitalization,hospitalization)of the observation group was significantly shorter than that of the control group(P<0.05).The rescue success rate of the observation group was significantly higher than the control group,and the incidence of VAP was lower than that of the control group(P<0.05);there was no significant difference in the reintubation rate between the two groups of patients(P>0.05).Conclusion:Invasive sequential non-invasive mechanical ventilation treatment can more quickly control the patient’s condition,improve the rescue effect,and can reduce the occurrence of VAP,which has more application value.
