高频叠加常频通气在气管支架治疗合并气管狭窄的恶性食管气管瘘中的应用

摘　　要：目的:研究高频和常频叠加通气在硬质气管镜下气管支架治疗合并气管狭窄的恶性食管气管瘘中的应用。方法:90例合并气管狭窄的恶性食管气管瘘在硬质气管镜下行气管硅酮支架植入术患者,随机分为3组:高频叠加常频通气组,高频通气组,常频通气组。通过血氧饱和度和呼气末二氧化碳监测以及动脉血气分析三组患者肺部氧合和二氧化碳排除情况。结果:高频叠加常频通气组三项指标均在正常范围内,高频通气组氧合难维持,常频通气组二氧化碳潴留明显。结论:高频叠加常频通气在硬质气管镜下支架治疗合并气管狭窄的恶性食管气管瘘中安全可靠,既能维持氧合、又能促进二氧化碳排除。Objective:To study the application of high-frequency and normal-frequency superimposed ventilation in the treatment of malignant esophageal-tracheal fistula with tracheal stricture under rigid bronchoscope.Methods:Ninety patients with tracheal stenosis who underwent tracheal silicone stent implantation under rigid bronchoscope were randomly divided into three groups:high frequency superimposed normal frequency ventilation group,high frequency ventilation group,and normal frequency ventilation group.The oxygenation and carbon dioxide elimination of the three groups were analyzed by blood oxygen saturation and endexpiratory carbon dioxide monitoring and arterial blood gas analysis.Results:The three indexes in the high frequency and normal frequency ventilation groups were all within the normal range.The oxygenation in the high frequency ventilation group was difficult to maintain,and the carbon dioxide retention in the normal frequency ventilation group was obvious.Conclusion:High-frequency superimposed normal-frequency ventilation is safe and reliable in the treatment of malignant esophageal and tracheal fistula with tracheal stenosis under rigid bronchoscopy.It can maintain oxygenation and promote carbon dioxide elimination.
