维持性血液透析患者舌象与血液透析前血肌酐水平的相关性

摘　　要：目的:探讨维持性血液透析(maintenance hemodialysis,MHD)患者中医舌象与血肌酐之间是否存在相关性。方法:应用舌象仪采集MHD患者透析前舌象图片,并导入《中医舌诊专家系统》,由3位主任医师对舌象各指标赋值,同时留取拍摄当日患者透析前静脉血以测定透析前血肌酐含量,寻找舌象参数与血肌酐的相关性。结果:与透析前血肌酐水平比较,差异具有统计学意义的舌象参数共有4个(P<0.05),分别为齿痕程度、水滑程度、紫舌、厚苔(P=0.024、0.008、0.023、0.048),其中齿痕程度与透析前血肌酐关系最密切,呈正相关。结论:部分舌象参数可在一定程度上反映MHD患者血肌酐蓄积程度,即当MHD患者齿痕越重、水滑程度越轻、舌色越紫、舌苔越薄时,其血肌酐可能处于偏高水平。Objective:To investigate whether there is a correlation between TCM tongue image and blood creatinine in maintenance hemodialysis(MHD)patients.Methods:Tongue imager was used to collect pictures of tongue images before dialysis in MHD patients,and it was imported into Tongue Expert System for Traditional Chinese Medicine.Three chief physicians assigned values to tongue images,and at the same time retained venous blood of patients on the day of shooting to determine dialysis before blood creatinine content,look for the correlation between tongue image parameters and blood creatinine.Results:Compared with the level of blood creatinine before dialysis,there were 4 tongue parameters with statistical significance(P<0.05),which were the degree of tooth marks,water slip,purple tongue and thick fur(P=0.024,0.008,0.023,0.048),in which the degree of tooth marks is the most closely related to the blood creatinine before penetration.Conclusion:Some tongue image parameters can reflect the degree of blood creatinine accumulation in patients with MHD to a certain extent.That is to say,when the dent marks are heavier,the degree of water slip is lighter,the tongue color is purple,and the tongue coating is thinner,the serum creatinine may be at a high level.
