复方丹参滴丸联合穴位埋线对早期糖尿病肾病瘀证患者尿微量白蛋白与肌酐比值的影响

摘　　要：目的:观察复方丹参滴丸联合穴位埋线对早期糖尿病肾病瘀证患者的尿微量白蛋白与肌酐比值(microalbuminuria/creatinine,ACR)的影响,并观察其持续疗效。方法:90例早期糖尿病肾病瘀证患者随机分为3组,每组各30例,分别为研究组(糖尿病常规治疗+大剂量复方丹参滴丸+穴位埋线)、对照组(糖尿病常规治疗+厄贝沙坦片)、空白组(糖尿病常规治疗),治疗12周,随访6个月,每3个月随访1次。结果:治疗后,与其他两组比较,研究组瘀证评分明显下降(P<0.01);与其他两组比较,研究组的ACR下降得更早,下降更明显(P<0.01)。随访6个月发现,研究组及对照组ACR水平维持平稳,空白组ACR有所回升,且研究组ACR水平显著低于其他两组,差异有统计学意义(P<0.01)。治疗后,研究组超敏C反应蛋白、肿瘤坏死因子、同型半胱氨酸水平均显著低于治疗前(P<0.01);研究组治疗后的胰高血糖素样肽-1水平高于治疗前(P<0.01)。结论:复方丹参滴丸联合穴位埋线治疗早期糖尿病肾病瘀证患者疗效显著,能降低ACR,且疗效持续;这可能与相关炎症因子、同型半胱氨酸下降,胰高血糖素样肽-1水平上升有关。Objective:To observe the effect of compound Danshen dripping pills combined with acupoint catgut embedding on microalbuminuria/creatinine(ACR) ratio in patients with early diabetic nephropathy and stasis syndrome,and observe its continuous efficacy.Methods:90 patients with early diabetic nephropathy and stasis syndrome were randomly divided into 3 groups,30 cases in each group,which were the research group(conventional treatment of diabetes+high-dose compound Danshen Pills+acupoint embedding) and the control group(conventional treatment of diabetes + Irbesartan tablets),blank group(conventional treatment of diabetes),treated for 12 weeks,followed up for 6 months,once every 3 months.Results:After treatment,compared with the other two groups,the stasis syndrome score of the study group decreased significantly(P<0.01);compared with the other two groups,the ACR of the study group decreased earlier and more significantly(P<0.01).After 6 months of follow-up,it was found that the ACR level of the study group and the control group remained stable,the ACR of the blank group rebounded,and the ACR level of the study group was significantly lower than that of the other two groups.After treatment,the hypersensitivity C-reactive protein,tumor necrosis factor,and homocysteine in the study group were significantly lower than before treatment(P<0.01);the level of glucagon-like peptide-1 in the study group after treatment was higher than before treatment(P<0.01).Conclusion:Compound Danshen Pills combined with catgut embedding at acupoints is effective in the treatment of early diabetic nephropathy patients with stasis syndrome,which can reduce ACR,and the efficacy continues.The level rises.
