揿针拔伸法联合手法推拿治疗颈源性眩晕

摘　　要：目的:探究揿针拔伸法联合手法推拿对颈源性眩晕患者颈源性眩晕症状与功能评分、颈椎活动度及脑动脉血流动力学的影响。方法:选择119例颈源性眩晕患者采用前瞻性研究方法,根据治疗方式不同分为对照组59例与观察组60例。对照组采用常规疗法治疗,观察组采用揿针拔伸法联合手法推拿治疗,比较两组患者颈源性眩晕症状与功能评估量表(cervical vertigo symptom and function evaluation scale,ESCV)评分、颈椎活动度及脑动脉血流动力学变化情况。结果:治疗后两组ESCV评分明显升高,与治疗前比较,差异有统计学意义(P<0.05),观察组优于对照组,差异有统计学意义(P<0.05);治疗后两组患者颈椎活动度均明显提高,与治疗前比较,差异有统计学意义(P<0.05),观察组治疗后颈椎活动度高于对照组,差异有统计学意义(P<0.05);治疗后两组患者大脑椎动脉平均流速、大脑基底动脉平均流速均明显升高,与治疗前比较,差异有统计学意义(P<0.05),治疗后观察组优于对照组,差异有统计学意义(P<0.05)。结论:揿针拔伸法联合手法推拿治疗颈源性眩晕患者可有效改善患者症状、功能,增加颈椎活动度,提升脑动脉血流动力学,改善大脑后循环。Objective:To explore the effects of the Qin Zhen Ba Shen method combined with Tuina manipulation on cervical vertigo symptoms and function scores,cervical spine activity,and cerebral arterial hemodynamics in patients with cervical vertigo.Methods:A total of 119 patients with cervical vertigo were selected using a prospective study method.According to different treatment methods,59 cases were divided into control group and 60 cases in observation group.The control group was treated with conventional therapies,and the observation group was treated with Qin Zhen Ba Shen method combined with Tuina manipulation.The cervical vertigo symptom and function evaluation scale(ESCV)score,cervical spine activity,and Changes in cerebral arterial hemodynamics.Results:The ESCV scores of the two groups increased significantly after treatment.Compared with before treatment,the difference was statistically significant(P<0.05).The observation group was better than the control group with a significant difference(P<0.05).Patients'cervical spine activity was significantly improved.Compared with before treatment,the difference was statistically significant(P<0.05).The cervical spine activity of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The average velocity of cerebral vertebral artery and average velocity of cerebral basal artery were significantly increased in the group.Compared with before treatment,the difference was statistically sig-nificant(P<0.05).After treatment,the observation group was better than the control group,the difference was statistically significant(P<0.05).Conclusion:The treatment of patients with Qin Zhen Ba Shen method combined with Tuina manipulation can effectively improve the symptoms and functions of patients,increase the cervical spine mobility,improve cerebral hemodynamics,and improve the posterior cerebral circulation.
