摘　　要：目的总结非那雄胺联合经尿道柱状水囊前列腺扩开术(Transurethral columnar balloon dilation of prostate,TCBDP)治疗小体积(<80 ml)前列腺增生的临床疗效以及手术体会。方法回顾性分析2017年6月至2018年6月收治的40例前列腺增生患者,术前口服非那雄胺5 mg,2次/d,7 d后行经尿道柱状水囊前列腺扩开术。术后随访2年,对该药物联合手术的方法进行有效性和安全性评价。结果本组40例患者手术时间18.5~40.5 min;术后随访率100%,无严重手术并症;发生1例严重血尿,1例暂时性尿失禁。治疗后患者国际前列腺症状评分(IPSS)、最大尿流率(Qmax)、膀胱残余尿量(PVR)、生活质量评分(QOL)均得到明显改善,治疗前后比较差异均有统计学意义(均P<0.05)。结论非那雄胺联合经尿道柱状水囊前列腺扩开术治疗小体积前列腺增生临床疗效显著,术中出血少,术后无严重出血,该方法更适用于小体积前列腺增生患者,具有一定的临床推广价值。Objective To summarize the clinical effect and experience of finasteride combined with transurethral columnar balloon dilation of prostate(TCBDP)in the treatment of small volume(<80 ml)benign prostatic hyperplasia(BPH).Methods 40 cases of BPH admitted from June 2017 to June 2018 were analyzed retrospectively.Finasteride(5 mg)was orally taken twice a day before the operation,and TCBDP was performed 7 days later.The patients were followed up for 2 years to evaluate the efficacy and safety of the combined therapy.Results The operation time was 18.5-40.5 minutes,the postoperative follow up rate was 100%,without serious operation complications,there were one case of serious hematuria and one case of temporary urinary incontinence.The international prostate symptom score(IPSS),maximum urine flow rate(Qmax),residual urine volume,quality of life all improved significantly,with statistically significant differences before and after treatment(all P<0.05).Conclusions Finasteride combined with TCBDP has a significant clinical effect in the treatment of small volume BPH,with less bleeding during the operation and no serious bleeding after the operation.This method is more suitable for the patients with small volume BPH,and has a certain clinical promotion value.
