摘 要:目的:观察与探讨有创与无创序贯性机械通气治疗慢性阻塞性肺疾病(COPD)所致严重呼吸衰竭的疗效。方法:选取慢性阻塞性肺疾病所致严重呼吸衰竭患者60例,将其随机分为两组,对照组患者接受有创机械通气治疗,观察组患者接受有创与无创序贯性机械通气治疗,观察两组患者的治疗效果。结果:与治疗前相比,两组患者治疗后的PaO2、PaCO2以及SaO2指标均有显著改善,差异具有统计学意义(P<0.05);与对照组相比,观察组患者治疗后的PaO2、PaCO2以及SaO2指标改善程度显著较好,此外,观察组患者的住院时间短,再插管率与死亡率均较低,以上数据差异均具有统计学意义(P<0.05)。结论:有创与无创序贯性机械通气治疗COPD呼吸衰竭,有效降低了患者再插管率和死亡率,治疗效果良好,为临床提供了一种可行的治疗慢性阻塞性肺疾病所致严重呼吸衰竭的方法,值得推广使用。Objective To observe and explore the effect of invasive and non-invasive sequential mechanical ventilation on severe respiratory failure caused by chronic obstructive pulmonary disease.Method 60 patients with severe respiratory failure caused by chronic obstructive pulmonary disease treated in our hospital were randomly divided into two groups.The control group received invasive mechanical ventilation treatment,while the observation group received invasive and non-invasive sequential mechanical ventilation treatment.The therapeutic effect of the two groups was observed.Results Compared with before treatment,the PaO2,PaCO2 and SaO2 indexes of the two groups were significantly improved after treatment(P<0.05).Compared with the control group,the PaO2,PaCO2 and SaO2 indexes of the observation group were significantly improved.In addition,the hospitalization time of the observation group was shorter,and the reintubation rate and mortality rate were lower.The above data were compared between the two groups.The difference was statistically significant(P<0.05).Conclusion Invasive and non-invasive sequential mechanical ventilation can effectively reduce the reintubation rate and mortality rate of patients,and has a good therapeutic effect.It provides a feasible method for clinical treatment of severe respiratory failure caused by chronic obstructive pulmonary disease,and is worth popularizing.
