摘　　要：目的采用应答指导治疗策略(RGT)优化EBTB治疗方案,通过临床研究实现为患者提供精确治疗EBTB提供良好的基础。方法筛选2016年6月~2018年3月90例支气管结核病(EBTB)患者,随机分成治疗组(n=45)和对照组(n=45)。对照组按照传统固定治疗方案进行治疗;治疗组采用RGT优化EBTB治疗。根据对比患者临床表现、胸部X线检查、支气管镜检查、痰转阴率的测定结果,统计有效率。并根据RGT策略EBTB治疗应答的定义,根据治疗结果统计应答结果。结果治疗后,治疗组的临床表现、胸部X线检查、支气管镜检查、痰转阴有效率均高于对照组,差异有统计学意义(P<0.05)。RGT策略EBTB治疗应答的比较结果也显示,治疗组的各项应答指数均显著高于对照组(P<0.05)。结论早期应答模式可以预测SER,应答模式为调整疗程提供依据,调整疗程可以获得更高的SER率。RGT策略有助于优化EBTB治疗,获得更好的疗效,可让EBTB患者得到及时、精准的治疗,获取最大的临床效果和根治EBTB。Objective To optimize the treatment plan of EBTB by response-guided treatment strategy(RGT),and to provide a good basis for accurate treatment of EBTB through clinical research.Methods 90 patients with bronchial tuberculosis(EBTB)from June 2016 to March 2018 were screened and randomized into treatment group(n=45)and control group(n=45).The control group was treated according to the traditional fixed treatment regimen.The treatment group was treated with RGT to optimize EBTB.According to the comparison of the patient's clinical manifestations,chest X-ray examination,bronchoscopy,and group sputum conversion rate,the effectiveness was analyzed.Based on the definition of the RGT strategy EBTB treatment response,the response results were statistically analyzed based on the treatment outcome.Results After treatment,the effectiveness of clinical manifestations,chest X-ray,bronchoscopy,and sputum conversion in the treatment group was higher than that in the control group(P<0.05).The comparison of RGT strategy EBTB treatment response also showed that the response scores of the treatment group were significantly higher than those of the control group(P<0.05).Conclusion The early response mode can predict SER,and the response mode provides a basis for adjusting the course of treatment.A higher SER rate can be obtained by adjusting the course of treatment.The RGT strategy helps to optimize the treatment of EBTB,achieve better results,and enable EBTB patients to receive timely and accurate treatment for maximum clinical effect and eradication of EBTB.
