摘 要:目的探讨早期干预对早产儿神经心理发育的影响。方法将2018年6月至2019年10月在兰州大学第一医院儿童医学保健中心体检的106例早产儿作为研究对象,根据是否行早期干预(运动、语言、认知、情感交流、社会交往能力训练)分为干预组(n=54)与未干预组(n=52),评估纠正月龄为2、3、4月龄时早产儿20项神经运动筛查通过率,并在纠正月龄为6、12月时进行Gesell发育量表五个能区(包括粗动作、精细动作、适应性行为、语言及个人-社会行为)发育商(DQ)测试。结果干预组与未干预组性别、胎龄、出生体重、新生儿期疾病、母亲妊娠年龄、剖宫产、围生期母体疾病、带养人学历、家庭月收入比较差异无统计学意义(P>0.05)。在纠正月龄2、3、4月龄时,干预组20项神经运动筛查通过率均高于未干预组[18.5%(10/54)比5.8%(3/52),44.4%(24/54)比15.4%(8/52),79.6%(43/54)比42.3%(22/52)](P<0.05)。纠正月龄为6月龄时,干预组Gesell发育量表五个能区(粗动作、精细动作、适应性行为、语言、个人-社会行为)DQ值均高于未干预组[(78.2±8.1)比(90.3±5.6)、(77.0±8.9)比(88.4±6.2)、(77.6±10.0)比(89.3±6.1)、(80.4±11.2)比(91.2±5.8)、(79.6±9.7)比(92.0±7.2)](P<0.05);纠正月龄为12月龄时,干预组Gesell发育量表五个能区DQ值均高于未干预组[(81.4±7.1)比(91.6±6.0)、(79.8±8.3)比(92.1±6.4)、(81.2±8.7)比(91.8±5.7)、(80.9±8.7)比(92.0±5.6)、(82.8±7.8)比(92.8±5.9)](P<0.05)。结论早期干预有助于改善早产儿的神经心理发育,临床工作中应重视对早产儿家长的早期干预指导,并对早产儿神经发育进行评估。Objective To explore the effect of early intervention on the neuropsychological development of premature infants.Methods From Jun.2018 to Oct.2019,106 preterm infants who were examined in the Children′s Medical and Health Care Center of the First Hospital of Lanzhou University were divided into an intervention group(n=54)and a non-intervention group(n=52)according to whether early intervention(exercise,language,cognition,emotional communication and social interaction ability training)was performed.The pass rate of 20 neuromotor screening items was evaluated for preterm infants at the corrected month age of 2,3 and 4 months,and developmental quotient(DQ)test of five energy areas of Gesell development scale(including rough action,fine action,adaptive behavior,language and personal social behavior)were conducted at the corrected month age of 6 and 12 months.Results There was no significant difference between the intervention group and the non-intervention group in gender,gestational age,birth weight,neonatal diseases,maternal gestational age,delivery mode,perinatal maternal disease,education background of the foster and family monthly income(P>0.05).At the corrected month age of 2,3 and 4 months,the pass rate of 20 items of neuromotor screening in the intervention group was higher than that in the non-intervention group[18.5%(10/54)vs 5.8%(3/52),44.4%(24/54)vs 15.4%(8/52),79.6%(43/54)vs 42.3%(22/52)](P<0.05).At the corrected month age of 6 months,the DQ values of the five regions of Gesell development scale(rough action,fine action,adaptive behavior,language,personal social behavior)in the intervention group were higher than those in the non-intervention group[(78.2±8.1)vs(90.3±5.6),(77.0±8.9)vs(88.4±6.2),(77.6±10.0)vs(89.3±6.1),(80.4±11.2)vs(91.2±5.8),(79.6±9.7)vs(92.0±7.2)](P<0.05);at the corrected month age of 12 months,the DQ values of the five energy areas of Gesell development scale in the intervention group were higher than those in the non-intervention group[(81.4±7.1)vs(91.6±6.0),(79.8±8.3)vs
