益肺补气方联合孟鲁司特钠治疗老年人支气管哮喘的疗效观察及对血清炎性因子的影响

摘　　要：目的 探讨益肺补气方联合孟鲁司特钠治疗老年人支气管哮喘的疗效及对患者血清肿瘤坏死因子α(TNF-α)、白细胞介素10(IL-10)、白细胞介素18(IL-18)的影响。方法 选取衢州市第二人民医院2017年3月至2019年3月收治的老年支气管哮喘患者62例,按照随机数字表法分为观察组和对照组各31例。两组在常规治疗的基础上,观察组采用益肺补气方联合孟鲁司特钠片治疗,对照组采用孟鲁司特钠治疗。两组疗程均为3个月。比较两组治疗疗效,治疗前后主要症状积分、TNF-α、IL-10、IL-18变化及不良反应发生情况。结果 观察组总有效率(96.77%,30/31)高于对照组(74.19%,23/31)(χ2=4.679,P<0.05);观察组治疗后喘息[(1.02±0.26)分]、咳嗽[(0.73±0.18)分]、胸闷气喘[(0.69±0.21)分]积分均低于对照组[(1.85±0.32)分、(1.64±0.27)分、(1.38±0.34)分](t=11.208、15.614、9.613,均P<0.05);观察组治疗后血清TNF-α[(105.46±13.19)ng/L]、IL-18[(169.84±16.47)ng/L]均低于对照组[(148.78±17.56)ng/L、(248.73±26.57)ng/L],而观察组IL-10[(36.57±6.48)ng/L]高于对照组[(28.25±5.67)ng/L],两组差异均有统计学意义(t=10.982、5.380、14.051,均P<0.05)。结论 益肺补气法联合孟鲁司特钠治疗老年人支气管哮喘疗效明显,可减轻患者症状及炎性反应。Objective To investigate the effect of Yifei Buqi recipe combined with montelukast sodium on elderly patients with bronchial asthma and its influence on serum levels of tumor necrosis factor-alpha(TNF-α),interleukin 10(IL-10)and interleukin 18(IL-18).Methods From March 2017 to March 2019,62 elderly patients with bronchial asthma admitted to the Second People's Hospital of Quzhou were randomly divided into observation group and control group according to the random number table method,with 31 cases in each group.The patients in the observation group were treated with Yifei Buqi recipe combined with montelukast sodium tablets,while the patients in the control group were treated with montelukast sodium tablets.Both two groups were treated for 3 months.The therapeutic effect,main symptom scores,changes of pulmonary function and inflammatory mediators,and adverse reactions were compared between the two groups before and after treatment.Results The total effective rate of the observation group[96.77%(30/31)]was higher than that of the control group[74.19%(23/31)](χ2=4.679,P<0.05).After treatment,the scores of asthma[(1.02±0.26)points],cough[(0.73±0.18)points]and chest tightness asthma[(0.69±0.21)points]in the observation group were lower than those in the control group[(1.85±0.32)points,(1.64±0.27)points and(1.38±0.34)points](t=11.208,15.614,9.613,all P<0.05).After treatment,the serum levels of TNF-α[(105.46±13.19)ng/L]and IL-18[(169.84±16.47)ng/L]in the observation group were lower than those in the control group[(148.78±17.56)ng/L and(248.73±26.57)ng/L],while IL-10 level[(36.57±6.48)ng/L]in the observation group was higher than that in the control group[(28.25±5.67)ng/L](t=10.982,5.380,14.051,all P<0.05).Conclusion Yifei Buqi recipe combined with montelukast sodium has obvious curative effect on elderly patients with bronchial asthma.It can alleviate symptoms,alleviate inflammation,with less adverse reactions and good drug safety.
