葛根定眩胶囊治疗颈交感神经源性颈性眩晕临床观察

摘　　要：目的观察葛根定眩胶囊对颈交感神经源性颈性眩晕临床症状的影响。方法随机将患者分为葛根定眩胶囊组(中药组)和盐酸氟桂利嗪胶囊组(西药组),观察治疗2周后疗效及症状功能评分、中医症状及2个月后的复发率。结果经过2周的治疗,中药组与西药组总体疗效比较有差异(P<0.05),中药组优于西药组;两组治疗前后症状积分比较均有显著性差异(P<0.01),治疗后中药组症状评分提高优于西药组(P<0.05);中医症状方面中药组优于西药组,其中,眩晕、耳鸣方面比较差异有统计学意义(P<0.05),颈项疼痛、头痛方面比较有显著差异(P<0.01);2个月后随访,中药组复发率低于西药组(P<0.05)。结论中药组能够显著缓解眩晕、颈肩强痛、恶心呕吐、头痛、耳鸣等临床症状,显著改善患者症状与功能评分,推测可能与葛根定眩胶囊具有解肌舒筋通络、温通太阳经脉、疏利颈部筋肉脉络、改善颈部软组织痉挛、降低颈部交感神经兴奋性功能,从而改善血管痉挛、增加血供有关。Objective To observe the effect of Gegen Dingxuan Capsule on the clinical symptoms of sympathetic neurogenic cervical vertigo.Methods Patients were randomly divided into Gegen Dingxuan Capsule group(traditional Chinese medicine group)and flunarizine hydrochloride capsule group(Western medicine group).To observe the curative effect,symptom function score and TCM symptoms after 2 weeks of treatment and recurrence rate after 2 months.Results After 2 weeks of treatment,there was a difference on terms of overall efficacy between the TCM group and the Western medicine group(P<0.05),and the TCM group was superior to the Western medicine group.There were significant differences in symptom scores before and after treatment between the two groups(P<0.01),and the improvement of symptom scores in the TCM group was better than that in the Western medicine group(P<0.05).Changes in TCM syndromes showed that the TCM group was better than the Western medicine group,among which,the differences of vertigo and tinnitus were statistically significant(P<0.05),and the neck pain and headache were significantly different(P<0.01).Follow-up after 2 months showed that the recurrence rate in the TCM group was lower than that in the Western medicine group(P<0.05).Conclusion Chinese medicine group can significantly relieve dizziness,neck shoulder pain,clinical symptoms such as nausea and vomiting,headache,tinnitus,significantly improve patients’symptoms and functional score,presumably with Gegen Dingxuan Capsule can relieve muscle,relax tendons and relax collaterals,dredge the meridians,unclog the neck brawn,and to improve the neck spasm of soft tissue,lower cervical sympathetic nerve excitability,thereby improving vascular spasm,increasing blood supply.
